Summary: This study demonstrated a significant association between thiopurine metabolite levels and infliximab trough levels in pediatric patients with Crohn's disease, suggesting that monitoring thiopurine metabolites is useful for optimizing combination therapy. Patients with undetectable 6-thioguanine levels were more likely to lose response to infliximab therapy. Additionally, suspected non-adherence to azathioprine therapy was found in 20% of patients.
