Summary: This study compared the effects of standard ventilation and a ventilatory strategy on the incidence of atelectasis during bronchoscopy under general anesthesia. The results showed that the ventilatory strategy significantly reduced the occurrence of atelectasis and had a sustained effect over time.

Summary: This study aimed to develop an artificial intelligence model robust to rotation and covering using video bronchoscopy images to improve accuracy in the anesthesiologic process. Through training and evaluation datasets, the EfficientNetB1 model was ultimately chosen, showing better performance in the evaluation dataset compared to most human experts.

Summary: Tracheal diverticulum is a rare occurrence that can cause difficulties in tube placement during tracheotomy. To prevent this complication, fiberoptic bronchoscopy should be used routinely for all patients undergoing tracheal intubation or tracheotomy.

Summary: This study aimed to assess the safety and efficacy of midazolam combined with different doses of alfentanil in DFB sedation. The results showed that higher doses of alfentanil combined with a fixed low dose of midazolam significantly reduced discomfort and improved patient satisfaction during DFB, without increasing the risk of desaturation.

Summary: This study aimed to develop a risk prediction model for long-term atelectasis in children with pneumonia. A retrospective study was conducted on 532 children and identified that the clinical course before bronchoscopy, length of stay, bronchial mucus plug formation, and age were independent risk factors for long-term atelectasis in children. The developed model demonstrated good predictive accuracy and clinical utility, providing valuable reference for the prevention and treatment of long-term atelectasis in children.

Summary: This article reports a rare case of endobronchial myelolipoma, in which the patient was treated with cryobiopsy and argon plasma coagulation (APC) and experienced symptom relief. The article highlights the importance of pathological biopsy in diagnosing endobronchial myelolipoma.

Summary: This study aimed to investigate the usefulness of Mycobacterium tuberculosis-polymerase chain reaction (MTB-PCR) with bronchial washing specimens in predicting the discontinuation of airborne infection isolation (AII) in hospitalized patients with suspected active pulmonary TB. The results showed that although a negative result from MTB-PCR with bronchial washing samples cannot exclude pulmonary TB, it can serve as a predictor for discontinuing AII.

Summary: Atelectasis is a common occurrence during surgery under general anesthesia, and recent studies have found a high incidence of up to 89% during bronchoscopy under general anesthesia. It can affect the accuracy of peripheral bronchoscopy, and efforts should be made to prevent its development when planning the procedure.

Summary: Flexible bronchoscopes are increasingly used in diagnostic and therapeutic medicine, reducing the need for general anesthesia. This case study presents a 2-year-old girl diagnosed with isolated pulmonary Langerhans cell histiocytosis using a cryoprobe via a flexible bronchoscope. The girl presented with weight loss, loss of appetite, and rapid breathing for 2 months, with examination findings including hypoxia, tachypnea, clubbing, and bilateral crepitations. Imaging studies revealed ground-glass opacities and cystic lesions in both lungs. Transbronchial cryobiopsy confirmed the diagnosis with CD1a staining. Flexible bronchoscopy with a cryoprobe is a useful tool for obtaining lung biopsy samples in children.

Summary: This study compared the clinical efficacy of propofol/remifentanil with propofol/esketamine for patient sedation during fiberoptic bronchoscopy. The combination of esketamine with propofol led to more stable intraoperative hemodynamics, lower dosage of propofol, lower transient hypoxia rate, fewer adverse events, and greater satisfaction among bronchoscopists compared to remifentanil.

Summary: Painless electronic bronchoscopy is valuable in diagnosing patients with atelectasis and may help clarify the etiology of atelectasis. Age, sex, and quantity of smoking may be clinical factors associated with atelectasis.

Summary: Although bronchoscopy with bronchoalveolar lavage (BAL) can provide useful information in characterizing severe asthma, its impact on patient management is limited in most cases. BAL may increase the identification of bacterial infections, particularly in nonatopic patients, but can decrease bronchoscopy tolerance, especially in the presence of tracheobronchial malacia or symptomatic patients.

Summary: Foreign body aspiration (FBA) occurs less frequently in adults than in children. Case reports of FBAs occurring in adults after lung surgery are not found in the literature, and this particular condition is often misdiagnosed. Medical history, careful physical examination, and fiberoptic bronchoscopic examination are important to exclude the risk of lung cancer recurrence and FBA after lung cancer surgery.
