Flow cytometry analysis of leukocytes in induced sputum from asthmatic patients

Inflammatory cell counts in induced sputum from asthmatic patients partially correlate with respiratory physiology data. To identify and quantify these inflammatory components, microscopy has been useful but it is not without its limitations. Flow cytometry could be an alternative but still has underlying methodological difficulties. While passing airways, leukocytes undergo morphologic cellular changes that alter their conventional phenotype. To demonstrate the usefulness of cytometry in accurately identifying cellular profiles in induced sputum of asthmatic and chronic cough patients, we introduced a new panel of monoclonal antibodies against specific subset markers. To identify neutrophils, sputum cells were stained with CD45 and CD66b. To identify eosinophils, sputum cells were stained with anti-CD45 and anti-CD125. We co-stained CD45, CD14 and CD66b to identify macrophages as CD45+CD14+CD66b- cells. Comparable results of trypan blue exclusion and annexin V-FITC suggested that cytometry manipulation did not decrease cellular viability. Range values were similar in microscopy neutrophils (median 19.9%, range 1.7-90.1%) and CD45+CD66b+ neutrophils (median 31% range 0.9-89%). After gating out CD45- non-leukocyte events, CD45+ and SSC dot-plots defined three patterns of leukocyte distribution. The eosinophil range in microscopic examination was 0-71.3% (median 2.85%) whereas CD45+CD125+ cell range in cytometry was 0-29% (median 3.7%). Since no exclusive markers were found on airways macrophages, we co-stained CD45, CD14 and CD66b to identify macrophages as CD45+CD14+CD66b- cells. Microscopy showed that macrophage and CD45+CD14+CD66b- cell counts were comparable (median 52.3 and range 6.7-94.8 vs median 61 and range 10.5-97.7 respectively). Correlations between neutrophils, eosinophils and macrophages in microscopic examination and flow cytometry were strong (R = 0.725, 0.747 and 0.532, respectively p < 0.001). This study validates effectiveness of combining specific antibodies and cytometry to quantify inflammatory leukocytes in induced sputum. Multiple markers at a single cell level will deepen our knowledge concerning the phenotype of airway leukocytes. (C) 2011 Elsevier GmbH. All rights reserved.

The immunoregulatory role of IL-35 in patients with interstitial lung disease

Summary: This study aims to investigate the immune regulatory role of IL-35 in the development of fibrosis in interstitial lung disease (ILD). The results suggest that IL-35 may induce an anti-fibrotic response, regulating the effect of TGF-beta and the inflammatory response on CD4+ T cells. Furthermore, the TGF-beta: IL-35 ratio in BAL has been shown to be a potential biomarker to predict the outcome of F patients with ILD.

Summary: MicroRNAs (miRNAs) are increasingly being used as biomarkers for diagnosis and treatment of various diseases. This study analyzed miRNA expression profiles in serum and lung tissue from severe asthma patients with and without oral corticosteroids (OCS) treatment. MiRNA-Seq and RT-qPCR were used for evaluation and validation of miRNA levels. Altered expression of miRNAs in serum and lung tissue was observed, with hsa-miR-221-5p and hsa-miR-769-5p showing the best differentiation between OCS-treated and untreated individuals. These miRNAs were found to be associated with important signaling pathways for asthma and regulated the expression of genes involved in these pathways. The findings suggest that miRNA profiles can serve as biomarkers for OCS treatment in asthma patients.

Characteristics of Induced-Sputum Inflammatory Phenotypes in Adults with Asthma: Predictors of Bronchial Eosinophilia

Summary: The purpose of this study was to determine the bronchial inflammatory phenotypes and associated characteristics in patients with asthma attending a specialist clinic. The study found that the most common phenotype was eosinophilic, and peripheral blood eosinophilia was the best predictor of bronchial eosinophilia. This information can help doctors select appropriate treatment and prevent asthma exacerbation.

Clinical Impact of Electronic Monitoring Devices of Inhalers in Adults with Asthma or COPD: A Systematic Review and Meta-Analysis

Summary: We conducted a systematic review and meta-analysis on electronic monitoring devices of inhalers (EMDs) in adult patients with asthma or COPD to examine their characteristics and clinical impact. Our search yielded eight observational studies and ten clinical trials, covering a wide range of clinical outcomes. The meta-analysis showed that adherence to inhalers improved in the EMD group over a three-month period. An exploratory analysis also found an improvement in ACT score. However, other clinical outcomes showed mixed results. Overall, EMDs have the potential to optimize adherence to inhaled therapy and may be of interest for other clinical outcomes.
