Summary: The definition of preeclampsia is changing to include organ symptoms alongside hypertension in pregnancy, rather than relying solely on proteinuria. Angiogenic and antiangiogenic factors have emerged as important tools for predicting and diagnosing preeclampsia with high accuracy. This review highlights their central role in the differential diagnosis of high-risk women and their ability to predict preeclampsia-associated adverse outcomes.

Summary: This study evaluated the predictive value of the sFlt-1/PlGF ratio in women with chronic hypertension for delivery due to superimposed preeclampsia, finding that angiogenic imbalance can predict delivery related to superimposed preeclampsia and is associated with a significantly shorter time to delivery interval.

Summary: This study evaluated the clinical characteristics and outcomes of pregnant women with chronic kidney disease (CKD) and suspected superimposed preeclampsia. The degree of angiogenic imbalance was found to be associated with disease severity and prognosis.

Summary: Superimposed preeclampsia complicates about 20% of pregnancies in women with chronic hypertension and is associated with increased maternal and perinatal morbidity. Biomarkers are being explored for screening, diagnosis, and risk stratification in these women, although current evidence suggests that existing biomarkers have limited discriminatory value compared with the general population.

Summary: This study compared the placental transcriptomes of pregnant women with hypertensive disorders to non-hypertensive controls and found significant differences in the transcriptome profiles of chronic hypertension superimposed on preeclampsia compared to preeclampsia without chronic hypertension. These findings suggest that preeclampsia superimposed on chronic hypertension may represent a distinct disease entity.

Summary: The study suggests that there may be an association between chronic hypertension and gut microbiome dysbiosis in pregnancy, which hinders gut microbial composition remodeling during superimposed preeclampsia.

Summary: This systematic review and meta-analysis aimed to investigate the potential benefits of low-dose aspirin use during pregnancy in women with chronic hypertension. The analysis found that low-dose aspirin did not significantly reduce the odds of superimposed preeclampsia or poor perinatal outcomes. However, it did show a significant reduction in preterm birth.

Summary: Hypertension complicates a significant percentage of pregnancies and is a leading cause of maternal and perinatal morbidity and mortality. This study aimed to evaluate the prognostic value of the soluble fms-like tyrosine kinase-1/placental growth factor ratio in women with suspected preeclampsia who have already met the diagnostic criteria. The findings suggest that this ratio is associated with an increased risk of developing preeclampsia with severe features.

Summary: Utilizing the SHRSP model, this study investigated placental mechanisms promoting superimposed preeclampsia in women with chronic hypertension. The findings revealed sustained activation of cellular stress response in SHRSP placentas, leading to redox imbalance and significant alterations in placental glycocode. The study highlights the potential role of glycoredox balance as a promising target for interventions in hypertensive disorders of pregnancy.

Summary: This study aimed to investigate the value of angiogenic marker assessment in women with chronic hypertension and suspected superimposed PE. The study found that adding angiogenic markers to the traditional diagnostic criteria significantly improved the prediction of adverse outcomes for both mothers and infants. Therefore, the assessment of angiogenic markers should be considered when evaluating pregnant women with chronic hypertension.

Summary: This study investigated the effects of chronic hypertension with superimposed preeclampsia on the structure and function of the human brain. It found that CHTN-PE patients showed significantly decreased gray matter volume (GMV) in the right middle temporal gyrus compared to the non-pregnant and pregnant healthy controls. This GMV decrease may be related to the decline in cognitive function in CHTN-PE patients.

Summary: This study tested the link between altered PlGF and sFLT-1 levels, and their ratio with PE and PE-associated features, and found that increased sFlt-1 levels and sFlt-1/PlGF ratio are independent risk factors for PE.

Summary: Hypertension poses a threat to society due to unknown causes, hindering proper management and affecting a growing number of patients. It is a high-risk factor for stroke, cardiac and renal complications, and disability. Soluble factors play an important role in the pathophysiology of hypertension, with potential neural effects that are not well understood in conditions such as preeclampsia. Shedding light on the central effect of these factors may advance our understanding and management of hypertension in preeclampsia.

Summary: Preeclampsia is a dangerous disorder for pregnant women and neonates that requires specific and reliable biomarkers for early prediction. The imbalance of angiogenic factors contributes to the pathophysiology. Applying a multivariable model from the start of pregnancy throughout gestation is essential for early and specific prediction of preeclampsia.

Summary: This study investigated the use of 1,3-butanediol in preventing preeclampsia in a rat model. The results showed that 1,3-butanediol significantly lowered blood pressure and improved uterine artery resistance in treated dams, with no adverse fetal effects observed. Physiological ketosis via 1,3-butanediol may be a potential therapeutic approach for managing hypertension and mitigating adverse pregnancy outcomes.
