Summary: This study used a claims database to examine the utilization of conservative interventions before hysterectomy in patients with uterine leiomyomas between 2011 and 2019. The results showed that 59.7% of patients did not receive any conservative intervention prior to hysterectomy. Factors associated with higher likelihood of receiving conservative interventions included more recent years of hysterectomy, presence of endometriosis, abnormal bleeding, greater comorbidity, or prior venous thromboembolism.

Summary: This study investigated the association between anogenital distance (AGD) and adenomyosis, endometriosis, and uterine leiomyomas (UL). The results showed that women with ovarian endometrioma (OE) and adenomyosis had significantly shorter AGD from the posterior fourchette to the center of the anus (AGDAF), but not from the anterior clitoral surface or the tip of the clitoris. The findings suggest that reduced prenatal exposure to androgens may increase the risk of developing endometriosis and adenomyosis, but other factors may also contribute to the pathogenesis of these conditions.

Summary: The aim of this study was to assess the prevalence of adenomyosis in symptomatic women in relation to the angle of flexion of the uterus. The study found that an angle of uterine flexion >210 degrees is associated with a higher prevalence of adenomyosis.

Summary: This study aims to evaluate pelvic organ prolapse-specific health outcomes and service utilization of women undergoing uterine suspension compared to those undergoing hysterectomy and vaginal vault suspension up to 1 year post-surgery. Data collection includes participant questionnaires, medical records, and administrative data linkage. The primary outcome is anatomical failure in correcting pelvic organ prolapse.

Summary: This review aims to evaluate cases of concurrent adenomyosis and congenital uterine anomalies (CUAs). The literature search retrieved 14 articles summarizing the findings regarding the concurrent diagnosis of adenomyosis and CUAs. Adenomyosis can be found in both syndromic and nonsyndromic CUAs, and the underlying mechanisms and contributing factors need further elucidation.

Summary: While most women with chronic pelvic pain experience significant pain relief following hysterectomy, higher levels of centralized pain before the surgery are a strong predictor of persistent pelvic pain in the long term.

Summary: Morcellation is a surgical technique used to reduce the size of the uterus or myomas, but it carries a risk of spreading malignancy in some cases. Therefore, preoperative evaluation to assess the risk of malignancy is crucial before considering morcellation for fibroids or the uterus.

Summary: Chronic pelvic pain is commonly associated with additional pathologies such as adenomyosis and endometriosis. A retrospective study found that 40% of patients with associated pathologies did not experience relief from pelvic pain at the early follow-up and 32% at the 12-month follow-up after laparoscopic niche repair. Therefore, careful patient selection is important for uterine scar repair in patients with concomitant adenomyosis and endometriosis.

Summary: This study aimed to investigate the prevalence of adenomyosis and its correlation with presurgical symptoms. The study found that bleeding disorders and dysmenorrhea were suspicious symptoms of adenomyosis. The incidence of adenomyosis in hysterectomy specimens exceeded 40%.

Summary: This systematic review evaluated the effect of procedural interventions for leiomyomas on pelvic floor symptoms. It found that urinary symptoms improved after surgical interventions for uterine leiomyomas, although there is high heterogeneity among studies and limited data on long-term outcomes or comparing different procedures.

Summary: This study aimed to improve the differential diagnosis performance of atypical focal adenomyosis and uterine leiomyomas using a contrast-enhanced ultrasound (CEUS) model. Qualitative and quantitative characteristics were analyzed, and a risk prediction model was established. The model showed high sensitivity and specificity in diagnosing the two diseases.

Summary: This study reveals an association between parity and chromosomal damage in uterine leiomyomas, suggesting that mechanical forces in a constrained cellular environment may contribute to tumor development without evidence of progression to malignancy.

Summary: This study investigated the use of monitored anesthesia care (MAC) to manage procedural anxiety in patients undergoing uterine artery embolization (UAE) for symptomatic uterine fibroids or adenomyosis. The results showed that MAC was successfully performed and significantly reduced bleeding and pain symptoms. 94.4% of women were satisfied with MAC in UAE. The study suggests that MAC is emotionally effective and safe for patients anxious about the procedure.

Summary: This study aimed to determine the incidence of patient-reported pain 1 year after benign hysterectomy for gynecological conditions and analyze the clinical risk factors for pain. Results showed that a significant proportion of women reported pelvic pain and new-onset pain after 1 year of surgery. Preoperative pain and postoperative complications were identified as risk factors for pelvic/lower abdominal pain after 1 year.

Summary: Hysterectomy increases the risk of pelvic organ prolapse surgery for nulliparous women by 60%. This historical matched cohort study found an increased risk of pelvic organ prolapse surgery after hysterectomy in comparison to women who did not undergo hysterectomy.
