Summary: This study investigates the effect of rFVIIa on bleeding in lung transplant patients undergoing ECMO therapy. Results show that only 14% of rFVIIa administrations were effective in controlling bleeding, while 71% of patients required surgery for bleeding control. Although 84% of recommended preconditions were fulfilled, their fulfillment was not associated with rFVIIa efficacy. The incidence of thromboembolic events after rFVIIa administration was comparable to patients not receiving rFVIIa.

Summary: DAH after HCT has a low incidence rate but high mortality rate, requiring attention. Multiple factors may influence the incidence rate, mortality rate, and risk factors, including time of onset, platelet count, and the need for invasive mechanical ventilation. For treatment outcomes, lower doses of steroids may help improve patient survival.

Summary: This study evaluated the use of nebulized tranexamic acid (TXA) in the treatment of post-tonsillectomy hemorrhage (PTH) and its association with rates of operative intervention. A retrospective cohort study was conducted on adult and pediatric patients diagnosed with PTH in 2015-2022 who were treated with nebulized TXA and standard care. The results showed that treatment with nebulized TXA was associated with lower rates of operative intervention and repeat bleeding events. Further prospective studies are needed to confirm the efficacy and optimal treatment protocols. Level of evidence: 3.

Summary: The study showed that erythropoietin (EPO) can alleviate diffuse alveolar hemorrhage (DAH) by reducing macrophage recruitment and promoting phenotype switch, suggesting its potential as a therapeutic agent for DAH. Further research is needed to validate the direct target of miR-494-3p in regulating JAK2/STAT3 signaling transduction.

Summary: Diffuse alveolar hemorrhage is a rare condition with diverse etiologies, where cardiac diseases are often underestimated causes, particularly heart failure and mitral valve diseases being the most common. DAH is typically caused by the stress failure of pulmonary capillaries due to pulmonary venous hypertension, with a contribution from the bronchial circulation.

Summary: Treating PTH with nebulized TXA may be a safe first-line therapy to reduce the need for operative control of bleeding. The data suggests that a large clinical trial is needed to determine the efficacy of nebulized TXA in mitigating this common and potentially fatal post-operative complication.

Summary: This case report highlights an uncommon presentation of delayed pulmonary hemorrhage after snakebite envenoming. The patient showed initial venom-induced consumption coagulopathy followed by diffuse alveolar hemorrhage (DAH), which was successfully treated with immunosuppressants and plasma exchange. Although persistent hypofibrinogenemia was observed, the patient remained healthy during the follow-up period.

Summary: The study found that Mivebresib effectively protected mice against pristane-induced diffuse alveolar hemorrhage (DAH) caused by systemic lupus erythematosus (SLE). It inhibited the migration and polarization of monocytes and macrophages, as well as pathogenic B and T cells. In addition, Mivebresib inhibited M1 polarization of macrophages and the expression of pro-inflammatory cytokines, suggesting its potential as a treatment for DAH.

Summary: This study evaluated the efficacy and safety of single-dose ketamine infusion in adults with sickle cell disease (SCD) who presented with acute sickle vasoocclusive crisis (VOC). The results showed that early use of ketamine resulted in a significant reduction in pain scores over a 2-hour period and reduced the cumulative morphine dose in the emergency department. Additionally, there were no significant drug-related side effects in the ketamine-treated group.

Summary: Secondary bacterial and fungal infections are common among COVID-19 patients admitted to the ICU, with a predominance of gram-negative bacteria and Candida species. The development of secondary infections is significantly associated with invasive mechanical ventilation. Summary: An updated systematic review and meta-analysis of ten studies found that the clinical efficacy of prophylactic inferior vena cava (IVC) filters in trauma patients for preventing pulmonary embolism (PE) remains uncertain. Observational studies showed a reduction in PE and fatal PE risk, while randomized controlled trials did not yield significant results. Therefore, current evidence does not support routine use of prophylactic IVC filters.

Summary: The aim of this study was to describe the frequency of candidemia cases, distribution of Candida spp., and the associated risk factors for mortality in an academic institution in Saudi Arabia. The study found that independent risk factors for mortality in candidemia patients included age, COVID-19 co-infection, and use of tocilizumab. A high rate of antifungal resistance was also observed, highlighting the importance of accurate laboratory testing.
