Summary: This study compared the risk of mortality between parathyroidectomy (PTx) and cinacalcet treatment in hemodialysis patients. The results showed that PTx resulted in greater reductions in intact PTH, calcium, and phosphorus levels compared with cinacalcet. PTx was associated with a lower risk of mortality, particularly among patients with severe secondary hyperparathyroidism.

Summary: This study investigated the relationship between erythropoiesis-stimulating agents (ESAs) consumption and parathyroidectomy (PTX) in end-stage renal disease (ESRD) patients with secondary hyperparathyroidism (SHPT), as well as the predictors for anemia improvement. The study found that PTX significantly reduced the requirement of ESAs, and patients with a low preoperative hemoglobin level or low transferrin saturation showed an increase in hemoglobin level after PTX.

Summary: This trial compared oral cinacalcet with total parathyroidectomy (PTx) in dialysis patients with advanced secondary hyperparathyroidism (SHPT). The results showed that both cinacalcet and PTx effectively improved biochemical abnormalities but did not significantly reduce cardiovascular surrogate outcomes or improve health-related quality of life. Cinacalcet may be used as an alternative to PTx for treating advanced SHPT.

Summary: This study analyzed the association between the prescription of active vitamin D and fracture incidence in hemodialysis patients. The results suggested that active vitamin D does not have a PTH-independent benefit in fracture prevention, supporting the current guideline of using active vitamin D only in patients with elevated or rising PTH.

Summary: This study observed the effects of different treatment modalities on blood pressure in MHD patients with secondary hyperparathyroidism. The results showed that parathyroidectomy significantly reduced blood pressure, while cinacalcet treatment significantly increased diastolic blood pressure.

Summary: This study investigated the relationship between COMMD5 levels and reduced blood pressure after PTx in patients receiving hemodialysis. The results showed that serum COMMD5 levels increased after PTx and COMMD5 promoted high expression of ANP and eNOS in endothelial cells, resulting in vasodilation and hypotension.

Summary: Persistent hyperparathyroidism after renal transplant negatively affects allograft function, but referral for definitive treatment of THPT is often delayed, leading to potential improvement in renal function with earlier referral.

Summary: Secondary hyperparathyroidism (SHPT) is common in patients with chronic kidney disease, and those who fail medical treatment should be referred to a parathyroid surgeon. Surgical indications, preoperative planning, intraoperative strategies, and postoperative management are crucial.

Summary: Ultrasound-guided percutaneous microwave ablation provides higher rates of achieving iPTH target levels compared to parathyroidectomy in patients with severe secondary hyperparathyroidism undergoing hemodialysis. Additionally, the microwave ablation group showed better performance in terms of iPTH target-achieving rates and decreases in iPTH concentrations.

Summary: This study retrospectively analyzed the outcomes of parathyroidectomy in patients with renal hyperparathyroidism, specifically looking at the influence of cinacalcet. The findings showed that cinacalcet did not impact the surgical outcomes and could be safely offered to patients who did not respond to medical treatment. Additionally, patients receiving cinacalcet had lower preoperative calcium levels, but there were no significant differences in other clinical parameters or complications compared to patients not on cinacalcet.

Summary: The meta-analysis shows that parathyroidectomy and cinacalcet treatment do not significantly affect graft function in patients with tertiary hyperparathyroidism. However, there is significant heterogeneity between studies, partially due to the lack of consensus in defining tertiary hyperparathyroidism, which limits the conclusions drawn from previously published series.

Summary: This retrospective study evaluated the efficacy of parathyroidectomy in patients with secondary hyperparathyroidism who underwent hemodialysis. The results showed that the surgery effectively improved biochemical indices, reduced intact parathyroid hormone levels, relieved clinical symptoms, and enhanced quality of life.

Summary: The use of natriuretic peptides for acute cardiac failure diagnosis is common among cardiologists and emergency-wards physicians, but controversial in chronic kidney disease patients. Clinical practices in interpreting natriuretic peptides, particularly B-type, play a crucial role in monitoring fluid variations and cardiac disease status in dialysis patients. Longitudinal follow-up of B-type natriuretic peptide is essential for early detection and treatment of extracellular fluid variations and cardiac disease status, which are associated with hard outcomes.

Summary: The study aims to investigate whether cholecalciferol supplementation can improve muscle strength in hemodialysis patients with low 25OHD levels. The main objective is to analyze the effect of oral cholecalciferol supplementation on muscle strength in these patients, while secondary objectives include analyzing 25OHD plasma levels after supplementation and factors associated with 25OHD decline speed, as well as assessing the impact on patient autonomy, frailty risk, and quality of life.

Summary: During the COVID crisis, NephroCare implemented necessary measures to ensure the safety of dialysis patients, which were modified during the three waves. These measures effectively controlled the dialysis parameters and PCR results of asymptomatic patients, with a contamination rate below the national average. In the third wave, vaccination was implemented, resulting in a decrease in COVID cases.

Summary: During the COVID-19 crisis, the Paris region was most affected, with 73% of all contaminations in French NC units occurring there. Dialysis frequency, processed blood volume, and other indicators decreased significantly in the Paris region but remained stable in other regions. The increase in 3-month weight loss among HD patients may be attributed to the cancellation of intradialytic snacks, affecting more malnourished patients or those with lower interdialytic weight gain.
