Midodrine for the Prevention of Vasovagal Syncope A Randomized Clinical Trial

Robert Sheldon, Peter Faris, Anthony Tang, Felix Ayala-Paredes, Juan Guzman, Manlio Marquez, Carlos A. Morillo, Andrew D. Krahn, Teresa Kus, Debbie Ritchie, Shahana Safdar, Connor Maxey, Satish R. Raj

Summary: Midodrine can reduce the recurrence of syncope in healthy, younger patients with a high syncope burden. The study showed fewer syncope episodes and longer time to first syncope in the midodrine group compared to the placebo group, with similar adverse effects in both groups.

Intracellular uptake of agents that block the hERG channel can confound the assessment of QT interval prolongation and arrhythmic risk

Alexander Burashnikov, Hector Barajas-Martinez, Robert Cox, Mark A. Demitrack, Michael J. Fossler, Michael Kramer, Robert B. Kleiman, Peter Kowey, Charles Antzelevitch

Summary: The study found that oliceridine and quinidine progressively prolonged the QTc interval and action potential duration in rabbit left ventricular wedge preparations over 5 hours, in line with slow tissue uptake of the drugs. Oliceridine caused modest prolongation of these parameters, while quinidine resulted in prominent prolongation and development of early afterdepolarization after 2 hours, leading to a high risk of torsades de pointes.

Epicardial ablation of incessant left atrial appendage tachycardia in a case with tachycardiomyopathy

Carla Losantos, David Barron, Manlio F. Marquez, Jorge Gomez, Moises Levinstein, Santiago Nava

Summary: Atrial tachycardias originating at the left atrial appendage are uncommon and may result in tachycardia-induced cardiomyopathy. In a case study of a 21-year-old female patient with incessant atrial tachycardia, successful RFCA at the epicardial aspect of the LAA resulted in symptom resolution and complete recovery of ventricular function.

Acacetin, a Potent Transient Outward Current Blocker, May Be a Novel Therapeutic for KCND3-Encoded Kv4.3 Gain-of-Function-Associated J-Wave Syndromes

Dan Ye, Wei Zhou, Samantha K. Hamrick, David J. Tester, C. S. John Kim, Hector Barajas-Martinez, Dan Hu, John R. Giudicessi, Charles Antzelevitch, Michael J. Ackerman

Summary: Based on the findings, the researchers suggest that Acacetin may be a novel therapeutic for patients with KCND3 gain-of-function-associated J wave syndrome by inhibiting Ito and abolishing the accentuated action potential notch in patient-derived iPSC-CMs.

Supraventricular tachyarrhythmia and sinus node dysfunction as a first manifestation of short QT syndrome in a pediatric patient. Case Report

Ruy G. Ploneda-Valencia, Willian A. Ortiz-Solis, Gustavo Ruiz-Gonzalez, Ana K. Santiago-Garcia, Leonardo Rivera-Rodriguez, Santiago Nava-Townsend, Manlio F. Marquez, Moises Levinstein-Jacinto

Summary: Short QT syndrome is a rare channelopathy characterized by a short QT interval on EKG and a tendency to develop arrhythmias. It can range from palpitations to sudden death. In this case report, a 16-year-old patient with short QT syndrome presented with sinus arrest and junctional rhythm, and later developed atrial tachycardia and atrial flutter.

Sudden Cardiac Arrest in Basketball and Soccer Stadiums, the Role of Automated External Defibrillators: A Review. For the BELTRAN Study (BaskEtbaLl and soccer sTadiums: Registry on Automatic exterNal defibrillators)

Mario D. Bassi, Juan M. Farina, Jorge Bombau, Mario Fitz Maurice, Guillermo Bortman, Elaine Nunez, Manlio Marquez, Norberto Bornancini, Adrian Baranchuk

Summary: This review aims to identify the risks and incidences of sudden cardiac arrest (SCA) and the use of automated external defibrillators (AEDs) in soccer and basketball stadiums. Athletes across all sports face an SCA risk of 1:50,000 athlete-years, with the highest risk in young male athletes (1:35,000 person-years) and black male athletes (1:18,000 person-years). AED use on-site improves survival rates, but many stadiums lack implementation of AEDs and face obstacles in their usage. Therefore, AEDs should be incorporated into stadiums' medical plans and have certified trained personnel for optimal utilization.

Jose M. Di Diego, Hector Barajas-Martinez, Robert Cox, Victoria M. M. Robinson, Joseph Jung, Mohamed Fouda, Mena Abdelsayed, Peter C. C. Ruben, Charles Antzelevitch

Summary: AR-787 has pleiotropic effects on cardiac ion channels and shows promise as a treatment for J wave syndromes and hypothermia.
