Summary: This study found that accelerated initiation of RRT can reduce the risk of all-cause mortality in surgical ICU patients and those receiving CRRT treatment, as well as increase the chances of eventual dialysis independence in these two subgroups. However, accelerated RRT initiation increases the risk of dialysis dependence in patients treated with non-CRRT methods or with high SOFA scores.

Summary: Through a service reconfiguration bundle, the number of peritoneal dialysis patients in Northern Ireland doubled, with a 97% success rate in starting PD. By using different techniques for PD catheter insertion, access to PD was expanded. This study highlights the effectiveness of flexible models of service delivery in increasing access to PD and home therapy.

Summary: When an adequate cephalic vein is not available for fistula construction, surgeons often turn to basilic vein or prosthetic constructions. We hypothesized that initial creation of a modest flow proximal forearm arterial-venous anastomosis to dilate (develop) inflow and outflow vessels, followed by a planned second-stage pro-cedure to create a cannulation zone with a prosthetic graft in the forearm, would result in reliable and durable hemo-dialysis access in patients with limited options.

Summary: This study investigated the impact of renal replacement therapy (RRT) modality and treatment protocol on RRT dependence in critically ill patients with acute kidney injury. The results showed that there was no difference in RRT dependence at day 28 between continuous RRT (CRRT) and intermittent hemodialysis (IHD). However, among survivors, CRRT-first was associated with decreased 28-day RRT dependence and more RRT-free days. Additionally, the ATN treatment protocol in CRRT-first patients was associated with increased RRT dependence and mortality.

Summary: Primary hyperoxaluria type 1 (PH1) is a rare genetic disorder characterized by excessive hepatic production of oxalate, leading to kidney stones and damage requiring renal replacement therapy (RRT). This study found that even with intensified RRT treatment, PH1 patients' plasma oxalate levels remained above target and showed increasing evidence of oxalate deposition. Echocardiographic data suggest potential cardiac dysfunction related to elevated oxalate levels in PH1 patients.
