Every cancer treatment, irrespective of its clinical effectiveness, has an impact on patients' quality of life (QoL). Even recently developed targeted therapies might have side effects and significantly impact patients' QoL. Thus, understanding the advantages and disadvantages of different treatments from the patient's standpoint has become a must in clinical research and is highly valued by major stakeholders. Thousands of cancer patients are enrolled into randomized controlled trials (RCTs) each year and many complete patient-reported outcome (PRO) instruments to obtain patient-centered information as part of the assessment of the overall effectiveness of the new therapy. Some of these RCTs have generated high quality PRO evidence forming the basis for approval (or support to approval) of drugs by the US Food and Drug Administration. However, a consistent strategy to determine the quality of patient centered evidence presented in RCTs has until recently been lacking. One of the fundamental questions when including PROs in clinical research revolves around methodological robustness and consistency of outcome reporting. Cancer patients, physicians and healthcare system stakeholders need to rely on solid information to make the best possible choice regarding treatment. Therefore generating high-quality findings from PRO assessment in cancer trials is of paramount importance. In an effort to improve quality of PRO assessment and reporting in the near future, the Patient-Reported Outcome Measurements Over Time In ONcology (PROMOTION) Registry was developed. The scope of this Registry is to identify, track, analyse, and store information on all cancer RCTs that have included PROs, and assess the quality of their PRO assessments.

Summary: This study aims to develop a COS, an agreed minimum set of outcomes to measure and report in studies evaluating novel surgical techniques. Currently, there is a lack of agreement on key outcomes for safe and efficient surgical innovation. The study generated a list of outcome domains, prioritized them through a Delphi survey, and reached consensus through a meeting involving international stakeholders.

Remote assessment of surgical site infection (SSI) using patient-taken wound images: Development and evaluation of a method for research and routine practice

Summary: This study developed and evaluated a method for patients to capture standardized wound images for remote wound assessment to detect surgical site infection (SSI). The results showed that the method is simple, standardized, and acceptable, and high-quality images can be used for remote assessment of SSI.

Patient-centred outcomes following non-operative treatment or appendicectomy for uncomplicated acute appendicitis in children

Summary: This study investigated the comparative patient-centered outcomes between non-operative and surgical treatment for uncomplicated acute appendicitis in children. The preliminary data showed that children treated with non-operative treatment reported higher short-term quality of life scores, shorter duration of requiring analgesia, more rapid return to normal activities, and shorter parental absence from work compared to those who underwent appendectomy. These findings suggest the importance of further measuring recovery profile and quality of life in a larger randomized controlled trial.

Development of a Bariatric Surgery Core Data Set for an International Registry

Summary: This study developed a Core Registry Set (CRS) - core data to include in bariatric surgery registries globally, which is the first step towards unifying bariatric surgery registries internationally. We recommend the CRS is included as a minimum dataset in all bariatric registries worldwide. Adoption of the CRS will enable meaningful international comparisons of bariatric operations.

Further considerations for placebo controls in surgical trials

Summary: The ASPIRE guidance, published in the Lancet in 2020, provided advice for the design and conduct of surgical trials with an invasive placebo control. Based on a more recent international expert workshop in June 2022, we now provide further insights into this topic, including the purpose and design of invasive placebo controls, patient information provision, and how findings from these trials may be used to inform decision-making.

Delineating COVID-19 subgroups using routine clinical data identifies distinct in-hospital outcomes

Summary: The COVID-19 pandemic has presented a significant challenge to healthcare systems worldwide. This study developed an unsupervised data-driven model called SuStaIn, which can be used to predict short-term infectious diseases like COVID-19 based on commonly recorded clinical measures. The model identified three COVID-19 subtypes and introduced disease severity stages, both of which were predictive of in-hospital mortality or escalation of treatment. This model can be adapted for future outbreaks of COVID-19 or other infectious diseases.

The framework consists of three key components: baseline data about modifications, details about modifications, and impact/consequences of modifications. This framework aims to support consistent and transparent reporting of modifications, fostering shared learning and incremental innovation of surgical procedures/devices.

Summary: The EBI programme did not significantly impact the use of the 17 procedures it targeted, as most procedures were already on a downward trend before the guidance was introduced. There was no evidence of changes in trend post EBI, nor any spill-over effects onto related procedures. Geographical variation in procedure rates across English CCGs remained stable both before and after EBI. Further research is needed to understand the barriers and enablers of de-adoption of inappropriate health care to support future efforts.

Summary: Between 2007 and 2020, only a small percentage of the primary care population eligible for WM referral or bariatric surgery based on national guidelines actually received either. Higher BMI and GP practice region showed the strongest associations with both outcomes. Regional disparities may indicate variations in the commissioning and provision of WM services nationwide.

Summary: This study evaluated the extent to which robot-assisted anti-reflux surgery (RA-ARS) has followed the IDEAL framework during its evolution. The results showed variability in the reporting standards of RA-ARS studies, with under-reporting of important aspects of study design and a high degree of outcome heterogeneity. Further well-designed prospective studies and randomized trials are needed, along with agreement on outcome selection, measurement, and reporting for future RA-ARS studies.
