Haplotype analysis confirms the association between the HCRTR2 gene and cluster headache

Background.-Several studies suggested that genetic factors play a role in cluster headache (CH) susceptibility. We found a significant association between the 1246 G > A polymorphism of the hypocretin receptor-2 (HCRTR2) gene and the disease. This association was confirmed in a large study from Germany but was not replicated in a dataset of CH patients from Northern Europe. Objective.-The purpose of this study was to further evaluate the association between CH and the HCRTR2 gene using new polymorphisms, estimating the frequency of different gene haplotypes, searching for gene mutations, and evaluating the effects of the examined polymorphisms on hypocretin binding sites. Methods.-We genotyped 109 CH patients and 211 healthy controls for 5 new polymorphisms of the HCRTR2 gene and we inferred different gene haplotypes. Complete HCRTR2 sequencing was undertaken for 11 independent CH patients, 5 of whom had a positive family history. The effects of the 1246 G > A polymorphism on the hypocretin binding sites were evaluated using different computer-assisted analyses. Results.-Three new polymorphisms of the HCRTR2 gene resulted significantly associated with CH. The GTAAGG haplotype resulted more frequent in cases than in controls (OR: 3.68; 95% CI: 1.85-7.67). No point mutation of the HCRTR2 gene was found. Binding analyses showed that the 1246 G > A polymorphism (substitution of valine at position 308 by isoleucine) has no effect on the hypocretin binding sites but could influence the dimerization process of the receptor. Conclusion.-Our data confirm previous studies suggesting that the HCRTR2 gene or a linked locus significantly modulates the risk for CH. In addition, we suggest that the V308I substitution of the HCRTR2 may interfere with the dimerization process of the receptor, thereby influencing its functional activity.

Summary: Orexins, hypothalamic neuropeptides that regulate various physiological functions, were found to have significantly increased concentrations in patients with behavioral variant frontotemporal dementia (bvFTD) and correlated with several clinical characteristics. Further larger studies are needed to confirm the evidence of orexinergic dysfunction in bvFTD and its relationship with clinical symptoms.

Summary: Positron emission tomography (PET) with F-18-Fluorodeoxyglucose (F-18-FDG) is essential for diagnosing dementia, and amyloid PET imaging is helpful for early detection of Alzheimer disease (AD). This study presents the first report of F-18-Florbetapir amyloid imaging in a 35-year-old male patient with early-onset Alzheimer disease (EOAD). The findings support the usefulness of amyloid PET imaging in diagnosing EOAD.

Cochlear Implant Results in Older Adults with Post-Lingual Deafness: The Role of Top-Down Neurocognitive Mechanisms

Summary: This prospective study aimed to investigate the cognitive and clinical predictors of audiological outcome in adult cochlear implant users. Results showed that younger age and better performance in the Verbal Semantic Fluency Test at baseline were associated with higher verbal recognition scores after one year of implantation. Age and patients' TMT-A performance at baseline were found to be the most significant predictors of audiological outcome.

Anxiety and depression in Alzheimer's disease: a systematic review of pathogenetic mechanisms and relation to cognitive decline

Summary: This study explores the potential relationship between anxiety, depression, and Alzheimer's disease (AD), as well as the cognitive impairment associated with AD. The findings suggest that biological factors, such as changes in neurotransmitters and brain structures, contribute to depression and anxiety in AD patients. Furthermore, it is proposed that emotional reactions to cognitive decline and coping difficulties may also play a role in the development of anxiety and depression in early-stage AD.

Summary: Through genetic association study, two missense variants on APOE were found to be associated with decreased AD risk, while two other variants on APOE were confirmed to be associated with increased AD risk. The location of these variants suggests the important role of the carboxyl-terminal portion of apoE in AD pathogenesis.

Investigating p62 Concentrations in Cerebrospinal Fluid of Patients with Dementia: A Potential Autophagy Biomarker In Vivo?

Summary: Several studies have shown that autophagy is defective in neurodegenerative disorders such as Alzheimer's disease and frontotemporal dementia. This study investigated the role of p62 in neurodegeneration by analyzing its concentrations in the cerebrospinal fluid (CSF) of AD and FTD patients. The results revealed increased levels of p62 in the CSF of both AD and FTD patients, suggesting its potential as a biomarker for these disorders.

Summary: This article discusses the incorporation of receptor occupancy data from healthy volunteers into a proof-of-concept study in patients, aiming to improve dose selection and decision-making based on available evidence. A minimal physiologically based pharmacokinetic model is used to model receptor occupancy and a Bayesian dynamic borrowing design is applied to supplement the study with informative prior distributions.
