ENDOSCOPIC HORIZONTAL PARTIAL LARYNGECTOMY BY CO2 LASER IN THE MANAGEMENT OF SUPRAGLOTTIC SQUAMOUS CELL CARCINOMA

Background. The objective of this study was to evaluate the results of endoscopic horizontal supraglottic laryngectomy (EHSL) by CO2 laser. Methods. Between 1996 and 2005, 78 patients underwent a horizontal supraglottic laryngectomy operation (HSL) with an external approach and 70 underwent laser EHSL, as treatment for supraglottic laryngeal squamous cell carcinoma (LSCC). We evaluated oncological endpoints, comparing the external and the endoscopic approach. Results. Among patients primarily treated by EHSL, the 5-year disease-specific survival (DSS) was 89% (vs 80% in the external approach group). Statistical analysis did not reveal significant differences between the 2 groups as for survival nor for organ preservation. The most significant clinical predictor for DSS is neck relapse (p < .0001). Conclusions. This study confirms the effectiveness of laser EHSL in which oncological outcome is similar to the external approach and functional results are probably better. Neck management in this setting is fundamental to warrant the best survival. (C) 2009 Wiley Periodicals, Inc. Head Neck 31: 1196-1206, 2009

However, its reliability and applicability in different populations, especially in de-intensification protocols, are debatable. This paper discusses its limits and suggests possible actions to overcome these limitations.

New standards for the management of nose vestibule malignancies

Summary: This study aims to define the current standard of care for nasal vestibule squamous cell carcinoma (NVSCC). Through reviewing recent acquisitions, it is found that the current staging system is inadequate. Current evidence suggests that brachytherapy is at least equivalent to surgery and superior to external beams in terms of oncological outcomes, and superior to both modalities in terms of cosmesis and function for the treatment of primary lesions without bone invasion.

Evaluation of Staging Systems for Cancer of the Nasal Vestibule

Summary: This retrospective study aimed to evaluate the staging systems for cancer of the nasal vestibule. The distribution of stages varied widely among the four evaluated systems, with the classification proposed by Bussu et al. showing the most balanced allocation of patients. The widespread adoption of a single system and the introduction of a designated topography code could improve data reporting and understanding of the disease. Further analysis is needed to determine the best classification system for nasal vestibule carcinoma.

The Role of BDNF as a Biomarker in Cognitive and Sensory Neurodegeneration

Summary: Brain-derived neurotrophic factor (BDNF) plays a crucial role in the central nervous system and sensory structures such as olfactory and auditory systems. It has been shown to have protective effects, promoting neuronal growth and survival, and modulating synaptic plasticity. However, conflicting data exist on BDNF expression and functions in the cochlear and olfactory structures. Studies suggest that BDNF levels may serve as a biomarker in neurodegenerative diseases, including Alzheimer's disease, hearing loss, and olfactory impairment. This review summarizes current research on BDNF in the brain and sensory domains, highlighting the potential of BDNF as a biomarker for early diagnosis and therapeutic target.

Adenotonsillectomy in Children with Obstructive Sleep Apnea Syndrome: Clinical and Functional Outcomes

Interventional Radiotherapy (Brachytherapy) for Nasal Vestibule: Novel Strategies to Prevent Side Effects

Summary: Integrational radiotherapy is an effective treatment for early stages nasal vestibule tumors, with high local control rates and low toxicity profiles. This study proposes novel strategies to reduce dose to organs at risk, including the use of metal shields for the eyes and the use of mouth swabs to push away the mandible. The results show statistically significant dose reduction for both strategies.

Summary: Chronic rhinosinusitis with nasal polyps (CRSwNP) has a significant impact on quality of life, and defining disease control is important for determining the best treatment strategy. However, disease control is not standardized, and a universally accepted tool is still missing. A nationwide survey of otorhinolaryngologists in Italy revealed that while international guidelines were known, their implementation was challenging. Clinical symptoms, particularly nasal obstruction, loss of sense of smell, and rhinorrhea, were considered important indicators of disease control.
