As long as it hasn't entered the "Under review" and "wait reviewer's scores" status, it hasn't been sent for external review. The "Under review" status is very short and may not always be visible. In fact, the "awaiting ae assigns reviewer" status means that the Associate Editor is still reviewing it and hasn't decided to send it for review yet. Once it is sent for review, if the number of reviewers accepting the review does not meet the minimum requirement, it will be under review. If it meets the minimum requirement, it will be in the "wait reviewer's scores" status.

Thank you for your reply. My first review took exactly two months and two days, while the second review took one and a half months. The AE (Associate Editor) was very efficient, as they recommended the first and second reviews in just one day. For the second review, my submission entered the waiting for scores status on the second day. The main delay was caused by the reviewers.

Early TMI used to have very classic articles about hardware systems, as well as classic reconstruction algorithms. However, in recent years, it has completely become a journal for Chinese people to publish DL + medical image articles. Our very innovative hardware work was delayed for 4 months by the AE and eventually rejected due to a small number of animals. We followed up with review reminders three times but to no avail. In the end, our work was snatched and published by colleagues, leaving us with no choice but to reluctantly submit to a lower impact factor journal. It is disappointing and disheartening.
