Initiating factors for the onset of OA: A systematic review of animal bone and cartilage pathology in OA

There is controversy over whether bone or cartilage is primarily involved in osteoarthritis (OA) pathogenesis; this is important for targeting early interventions. We explored evidence from animal models of knee OA by preforming a systematic review of PubMed, Scopus, and Web of Science for original articles reporting subchondral bone and cartilage pathology in animal models with epiphyseal closure. Extracted data included: method of induction; animal model; cartilage and bone assessment and method; meniscal assessment; skeletal maturity; controls; and time points assessed. Quality scoring was performed. The best evidence was synthesized from high-quality skeletally mature models, without direct trauma to tissues of interest and with multiple time points. Altogether, 2849 abstracts were reviewed. Forty-seven papers were included reporting eight different methods of inducing OA, six different species, six different methods of assessing cartilage, five different bone structural parameters, and four assessed meniscus as a potential initiator. Overall, the simultaneous onset of OA in cartilage and bone was reported in 82% of datasets, 16% reported bone onset, and 2% reported cartilage onset. No dataset containing meniscal data reported meniscal onset. However, using the best evidence synthesis (n = 8), five reported simultaneous onset when OA was induced, while three reported bone onset when OA occurred spontaneously; none reported cartilage onset. In summary, there is a paucity of well-designed studies in this area which makes the conclusions drawn conjectures rather than proven certainties. However, within the limitation of data quality, this review suggests that in animal models, the structural onset of knee OA occurs either in bone prior to cartilage pathology or simultaneously.

Impact of osteoarthritis disease severity on treatment patterns and healthcare resource use: analysis of real-world data

Bone Marrow Lesions and Magnetic Resonance Imaging-Detected Structural Abnormalities in Patients With Midfoot Pain and Osteoarthritis: A Cross-Sectional Study

Summary: This study compared MRI-detected structural abnormalities in patients with symptomatic midfoot osteoarthritis, patients with persistent midfoot pain, and asymptomatic controls. The findings suggest a prevalence of osteoarthritis in individuals with persistent midfoot pain but no signs of OA on radiography, particularly in specific joints. However, the MRI abnormalities were not strongly associated with pain or foot-related disability.

Enthesitis in patients with psoriatic arthritis treated with secukinumab or adalimumab: a post hoc analysis of the EXCEED study

Summary: This study evaluated the treatment response of enthesitis in PsA patients treated with secukinumab or adalimumab for 52 weeks. The results showed that both drugs had similar efficacy in resolving enthesitis, including time to resolution. The authors concluded that inhibition of IL-17 with secukinumab reduced clinical enthesitis similarly to TNF-alpha inhibition with adalimumab.

Summary: This study used an adaptive multicriteria decision analysis to determine the relative weights of items in the JAMRIS-SIJ scoring system for juvenile idiopathic arthritis. Experts completed a conjoint analysis survey, comparing hypothetical patient profiles to determine the point value of the measurement items. The study found differences in relative weights among the JAMRIS-SIJ measurement items and validated the weighted scoring system.

Characterization of adverse joint outcomes in patients with osteoarthritis treated with subcutaneous tanezumab

Summary: This study monitored patients with osteoarthritis who were treated with tanezumab and found that higher doses were associated with increased risk of RPOA, TJR, and CJSE. Patients with more severe symptoms of osteoarthritis were also at higher risk of RPOA. Patients with KL grade 4 joints at baseline had the highest risk of RPOA2 or TJR.

Summary: This study aimed to evaluate the efficacy and safety of the anti-catabolic ADAMTS-5 inhibitor S201086/GLPG1972 for the treatment of symptomatic knee osteoarthritis. The results showed that S201086/GLPG1972 did not significantly reduce rates of cartilage loss or modify symptoms in adults with knee osteoarthritis.

The usefulness of ultrasound in predicting outcomes in patients with shoulder pain: a prospective observational study

Summary: This study aimed to determine if groups of ultrasound-based shoulder pathologies exist and how they relate to treatment outcomes. The results showed that current ultrasound-based classification methods cannot predict medium-term treatment outcomes for shoulder pain. Therefore, the role of routine diagnostic ultrasound for shoulder pain needs to be considered, and evidence-based therapies for specific pathologies should be established.

A scoping review was conducted to identify outcome domains in the existing literature and feedback was obtained on the next steps of the project. The findings from the review and the feedback will contribute to the development of the core outcome set.
