Effects of metformin plus simvastatin on polycystic ovary syndrome: a prospective, randomized, double-blind, placebo-controlled study

Objective: To evaluate the effect of a combination of simvastatin and metformin on biochemical parameters in women with polycystic ovary syndrome (PCOS). Design: A prospective, randomized, double-blind, placebo-controlled study. Setting: University hospital. Patient(s): Eighty-four women with PCOS randomly divided to two study groups. Intervention(s): Patients were randomly assigned to receive metformin (500 mg three times a day) plus simvastatin (20 mg/day, n = 42; group 1) or metformin (500 mg three times a day) plus placebo (once a day, n 42; group 2) for 12 weeks. Blood samples were obtained before and after treatment. Main Outcome Measure(s): Testosterone. Result(s): After 12 weeks of treatment, serum T levels decreased by 25.5% in group 1 and by 16.8% in group 2. There was a greater decrease of LH (45.5% vs. 6.7%) and a greater decline of the LH/FSH ratio (38.3% decreased vs. 4.4% increased) in the first group. In group 1 there was a greater decrease of total cholesterol (29.5% vs. 4.2%), low-density lipoprotein (LDL; 18.5% vs. 1.5%), and triglycerides (32% vs. 5.3%). High-density lipoprotein (HDL) increased in the first group by 14%, whereas it decreased by 1% in the second group. Conclusion(s): This report has demonstrated that the combination of metformin and simvastatin could lead to a better reduction of T and LH levels and thus reversing the LH: FSH ratio, lipid profile, and insulin resistance in patients with PCOS and may be an appropriate management option for patients with PCOS. COVID-19 pneumonia and pregnancy; a systematic review and meta-analysis

Summary: This study conducted a meta-analysis on 87 SARS-CoV-2 positive pregnant women and found that almost 65% of the patients had a history of exposure to an infected person, 78% suffered from mild or moderate COVID-19 pneumonia, and 99.9% had successful termination. No evidence of vertical transmission was found. Despite the immunosuppressive state during pregnancy, most patients had mild or moderate COVID-19 pneumonia with no pregnancy loss.

The effect of mother's β-thalassemia minor on placental histology and neonatal outcomes

COVID-19 and maternal, fetal and neonatal mortality: a systematic review

Summary: This comprehensive review examines the available evidence on maternal, fetal, and neonatal mortality associated with COVID-19 infection. The results suggest that maternal mortality is more common in women with pre-existing conditions, while fetal and neonatal mortality may be a result of maternal infection severity or prematurity. However, further research is needed to determine the exact magnitude of these mortality rates.

Predictive value of vascular endothelial growth factor and placenta growth factor for placenta accreta spectrum

A valid cut-off point for PLGF was reported at 63.55, presenting high predictive value for PAS. This study highlights the importance of early screening using maternal serum PLGF levels to reduce maternal-foetal morbidity and mortality for PAS patients.

Vaginal misoprostol and intravenous oxytocin for success of termination in the second-trimester intrauterine fetal demise: A randomized controlled clinical trial

As the second-line treatment, both methods had the same success rate.

Effects of oral glibenclamide versus subcutaneous insulin on perinatal outcome of patients with gestational diabetes mellitus: A randomized clinical trial

Summary: The study compared the efficacy and safety of oral glibenclamide and subcutaneous insulin in patients with gestational diabetes mellitus. The results showed that oral glibenclamide was as effective and safe as subcutaneous insulin in glycemic control and maternal and neonatal outcomes. Therefore, it could be used as a first-line treatment for gestational diabetes mellitus.

Outcome of early versus late ileostomy closure in patients with rectal cancers undergoing low anterior resection: A prospective cohort study

Summary: This study compared the effects of early versus late closure of the stoma in patients with rectal cancer undergoing low anterior resection. The results showed that early closure was associated with shorter operation duration and lower intra-operative bleeding. There was no difference in complications between the two groups. Therefore, early closure of the stoma is a safe and feasible technique.

Effects of serum fibrinogen correction on outcome of traumatic cranial surgery: A randomized, single-blind, placebo-controlled clinical trial

Summary: The effectiveness of correcting serum fibrinogen level in severe traumatic brain injury patients undergoing traumatic cranial surgery was evaluated in this study. Early delivery of fibrinogen concentrate (FC) was found to decrease intraoperative bleeding, but had no significant effect on other parameters. Further multicenter studies are recommended to investigate the role of early FC administration in the management of post traumatic coagulopathy.

Summary: Intervertebral disc degeneration (IDD) is a major cause of low back pain and has significant social and economic impact. Overcoming obstacles associated with these therapies will allow for their translation from bench to bedside in the near future.

Effectiveness of pentoxifylline in severe early-onset fetal growth restriction: A randomized double-blinded clinical trial

Additionally, it reduced neonatal mortality by decreasing oxidative stress markers and inflammatory cascade.
