Urinary tract infections (UTIs) are conditions frequently complained by women both in the general population and in the hospital setting. Indeed it has been estimated that one woman out of three will experience at least an episode of UTI during lifetime. A comprehensive literature review of published experimental and clinical studies of UTI was carried out at the University of Insubria electronic library (SFX Bicocca-Insubria) with cross-search of seven different medical databases (AMED, BIOSIS Previews on Web of Knowledge, Cochrane Library, Embase and Medline on Web of Knowledge, OvidSP and PubMed). We aimed to draw a clinical guideline addressed to the management of UTI, based on the most recent evidence. (C) 2011 Elsevier Ireland Ltd. All rights reserved.

When does oasis cause de novo pelvic floor dysfunction? role of the surgeon's skills

Maurizio Serati, Alessandro Ferdinando Ruffolo, Chiara Scancarello, Andrea Braga, Stefano Salvatore, Fabio Ghezzi

Summary: This study aimed to investigate the impact of surgeon skills in repairing obstetric anal sphincter injuries (OASIS) on the incidence of pelvic floor dysfunction (PFD). The results showed that skilled surgeons had a protective effect against PFD. Additionally, operative delivery and severe OASIS were associated with PFD.

Summary: This study assessed the efficacy and safety of retro-pubic tension-free vaginal tape (TVT) 20 years after implantation for the treatment of female stress urinary incontinence (SUI). The results showed that TVT is a highly effective and safe option for the treatment of SUI, with no significant deterioration of subjective and objective cure rates over time. However, some patients experienced the onset of de novo overactive bladder (OAB) after 20 years of follow-up.

Acceptability and perceived value of urodynamics from the patient perspective: A narrative review

Maurizio Serati, Andrea Braga, Peter F. W. M. Rosier, Stefan de Wachter, Alan Uren, Enrico Finazzi-Agro

Summary: From the patient perspective, urodynamic evaluation (UDS) appears to be a well-accepted and well-tolerated diagnostic tool in patients with lower urinary tract symptoms, particularly when an appropriate explanation is provided before the examination. Patients value the objective information provided by UDS and that this outweighs the temporary invasiveness of the test. This information is particularly relevant in light of the relative lack of evidence in the literature about patient expectations of specialist care in functional urology, which may have hindered progress with quality of care.

Summary: In this study, a prognostic nomogram was developed to assess the risk of cervical dysplasia persistence/recurrence after primary conization. Variables such as diagnosis of CIN3, presence of high-risk HPV types, positive endocervical margins, HPV persistence, and omission of HPV vaccination after conization were found to significantly increase the risk of cervical dysplasia persistence/recurrence. The nomogram can be a useful tool for counseling women about their risk and the importance of HPV vaccination after conization.

How should prospective research be designed to legitimately assess the value of urodynamic studies in female urinary incontinence?

Tufan Tarcan, Enrico Finazzi-Agro, Thomas M. Kessler, Maurizio Serati, Eskinder Solomon, Peter F. W. M. Rosier

Summary: This article discusses the design considerations for evaluating the value of urodynamic studies (UDS) in functional urology, suggesting that assessing the value of diagnostic tests based solely on treatment success is conceptually flawed. The article also provides recommendations for the design of a hypothetical randomized controlled trial, including considerations for study population, clinical endpoints, intervention and control group design, among others.

Summary: This narrative review evaluated the efficacy and safety of new minimally invasive solutions for stress urinary incontinence (SUI) treatment, specifically the microablative fractional carbon dioxide laser and the non-ablative Erbium-YAG laser. The short-term follow-up results showed significant subjective improvement, although cure rates were low and longer-term effects require further investigation.

The Learning Curve of Urodynamics for the Evaluation of Lower Urinary Tract Symptoms

Matteo Frigerio, Marta Barba, Alice Cola, Silvia Volonte, Giuseppe Marino, Luca Regusci, Paola Sorice, Giovanni Ruggeri, Fabiana Castronovo, Maurizio Serati, Marco Torella, Andrea Braga

Summary: This study assessed the learning curve of residents with no previous experience in correctly performing and interpreting urodynamics, as well as managing patients with pelvic floor disorders based on urodynamic findings. The results showed that the number of exams performed was directly related to the duration of the procedure, perceived difficulty, need for consultant intervention, accuracy of interpretation, and appropriateness of the therapeutic proposal. It was found that completing a learning curve may require performing 5 uroflowmetries and 15 cystomanometries with pressure/flow studies.

Summary: The objective of this study was to assess the influence of the test-to-delivery interval on the performance of ultrasound and MRI in predicting birthweight. The results showed that MRI performed best in predicting birthweight greater than gestational age if delivery occurred within two weeks of the examination, with a slight decrease thereafter, while ultrasound performance decreased drastically over time.

Induction of labor versus expectant management in patients with idiopathic polyhydramnios

Summary: This retrospective cohort study aimed to evaluate whether induction of labor is associated with a lower risk of cesarean section in patients with isolated polyhydramnios. The study found that planned induction was associated with a lower rate of cesarean delivery compared to expectant management, but the difference was not statistically significant. Additionally, no differences were observed in maternal or fetal secondary outcomes.

Summary: This systematic review examined the comorbidity of idiopathic intracranial hypertension (IIH) and polycystic ovarian syndrome (PCOS) and their impact on metabolism, hormone levels, and reproduction. The findings showed a high prevalence of comorbidity between the two conditions, which may have significant clinical implications for patient management.
