A combined score of pro- and anti-inflammatory interleukins improves mortality prediction in severe sepsis

Identification of patients at increased risk of death is dramatically important in severe sepsis. Cytokines have been widely assessed as potential biomarkers in this disease, but none of the cytokines studied has evidenced a sufficient specificity or sensitivity to be routinely employed in clinical practice. In this pilot study, we profiled 17 immune mediators in the plasma of 29 consecutively recruited patients with severe sepsis or septic shock, during the first 24 h following admission to the ICU, by using a Bio-Plex Human Cytokine 17-Plex Panel (Bio-Rad). Patients were 66.1 year old in average. Twelve patients of our cohort died during hospitalization at the ICU, eight of them in the first 72 h due to multiorganic dysfunction syndrom (MODS). Levels in plasma of three pro-inflammatory mediators (IL-6, IL-8, MCP-1) and of an immunosuppressive one (IL-10) were higher in those patients with fatal outcome. We developed a combined score with those cytokines showing to better predict mortality in our cohort based on the results of Cox regression analysis. This way, IL-6, IL-8 and IL-10 were included in the score. Patients were split into two groups based on the percentile 75 (P75) of the plasma levels of these three interleukins. Those patients showing at least one interleukin value higher than P75 were given the value 1. Those patients showing IL-6, IL-8, IL-10 levels below P75 were given the value 0. Hazard ratios for mortality at day 3 and day 28th obtained with the combined score were 2-3-fold higher than those obtained with the individual interleukins values. In conclusion, we have described a combined cytokine score associated with a worse outcome in patients with sepsis, which may represent a new avenue to be explored for guiding treatment decisions in this disease. The accuracy of the Abbott ARCHITECT HCV Ag assay was evaluated in people living with HIV/AIDS (PLWHA), showing good performance in high-prevalence settings but poor in low-prevalence settings. It was excellent in excluding active HCV infection.

Relative telomere length impact on mortality of COVID-19: Sex differences

Telomere shortening is associated with higher risk of infections and may be used to identify those patients who are more likely to die. This study evaluated the association between relative telomere length (RTL) and COVID-19 mortality. Lower RTL was associated with a higher risk of death in women at 30 and 90 days.

PBMCs gene expression signature of advanced cirrhosis with high risk for clinically significant portal hypertension in HIV/HCV coinfected patients: A cross-control study

Summary: We identified a gene expression signature associated with advanced cirrhosis with high risk for clinically significant portal hypertension (CSPH) in HIV/HCV coinfected patients through analyzing the gene expression profile of peripheral blood mononuclear cells (PBMCs). A cross-sectional study on 68 patients was conducted, and liver stiffness measurement (LSM) was used for stratification. We found 60 differentially expressed transcripts between patients with LSM < 12.5 kPa and >= 25 kPa, and two of them were validated in a different cohort.

Predictive plasma biomarkers of long-term increase in hepatic steatosis index after HCV eradication in HIV/HCV-coinfected patients

Summary: This study aimed to evaluate the association between plasma immune checkpoint proteins at baseline with hepatic steatosis index increase at the end of follow-up in HIV/HCV-coinfected patients. The results showed that higher levels of immune checkpoint proteins BTLA, CD137, CD80, GITR, LAG-3, and PD-L1 before HCV therapy were associated with a long-term increase in hepatic steatosis index after successful HCV therapy, suggesting a potential predictive role for early detection of progression towards steatosis in HIV/HCV-coinfected patients.

Environmental factors are associated to hospital outcomes in COVID-19 patients during lockdown and post-lockdown in 2020: A nationwide study

Summary: This study investigated the relationship between short-term exposure to environmental factors and COVID-19 outcomes in Spain during the lockdown and post-lockdown period. The results showed that PM2.5 had the strongest association with hospital admissions and mortality, followed by PM10, NO2, and SO2. ICU admissions were also associated with these pollutants. PM2.5 and PM10 were the most critical factors related to ICU mortality.

Genotyping data from 9373 COVID-19-positive cases and 5943 population controls were analyzed. The study found associations between HLA-B*14:02 and HLA-C*08:02 alleles with a lower risk of COVID-19 infection, and HLA-A*11:01 and HLA-C*04:01 alleles with disease severity.

Production and characterization of monoclonal antibodies for the detection of the hepatitis C core antigen

Summary: The study aimed to produce murine monoclonal antibodies against HCVcAg for the rapid and inexpensive detection of HCV infection. Four specific monoclonal antibodies were obtained, with three of them being able to recognize HCVcAg from all genotypes tested. These antibodies showed stable binding properties and can be used for detecting HCVcAg in HCV-infected samples.

Summary: Macrophages play a significant role in the progression of cryptococcosis. This study investigated the potential involvement of macrophage-derived exosomes in the pathogenesis of cryptococcosis. The study analyzed the changes of microRNAs in macrophage exosomes and explored the effects of highly expressed exo-miRNAs on human endothelial cell permeability, ROS accumulation, and pyroptosis in bronchial epithelioid cells. The results showed that exo-miR-4449 plays a crucial role in the pathogenesis of cryptococcosis and holds promise as a significant biomarker for treatment.

Inflammatory marker levels in children with tobacco smoke exposure

Suppression of DNMT2/3 by proinflammatory cytokines inhibits CtBP1/ 2-dependent genes to promote the occurrence of atrophic nonunion

Summary: Failure of bone healing after fracture often results in nonunion, but the underlying mechanism of nonunion pathogenesis is poorly understood. This study reveals that the inflammatory microenvironment in atrophic nonunion inhibits the expression of DNA methyltransferases, resulting in overexpression of CtBP1/2. The increased CtBP1/2 acts as transcriptional corepressors that suppress the expression of six genes involved in bone healing. The disruption of this signaling may represent a new therapeutic strategy to prevent nonunion incidence after bone fracture.
