Cervical cancer has been a leading cause of morbidity and gynecologic cancer deaths throughout the world in this generation despite the implementation of Pap smears. The American Joint Committee on Cancer classifies metastatic cervical cancer as any tumor (T) stage and M1 (distant metastasis of peritoneal spread and involvement of supraclavicular, mediastinal, or para-aortic lymph nodes; lung; liver; or bone) at primary presentation or persistent/recurrent disease outside the pelvis. Radiation with platinum-based chemotherapy is the standard treatment for locally advanced and potentially curable disease at limited metastatic site(s). For patients with recurrent cervical cancer after definitive surgery who have not received prior radiotherapy, salvage chemoradiation is an option. Meanwhile, surgery may be offered to patients with resectable disease if they have received primary radiotherapy. Patients with distant relapse at sole/limited metastatic site(s) could undergo salvage treatment by chemoradiation, surgery plus radiotherapy/chemoradiation, or surgery alone to achieve prolonged survival; hence, they should not be treated with systemic therapy alone. For previously irradiated unresectable lesions or disseminated disease, no effective control of the disease is available; therefore, such patients are candidates for systemic treatment. The primary goal of chemotherapy for those who are not amenable to curative intent is to extend life while offering quality of life. Results of clinical trials using platinum/nonplatinum doublets, molecularly targeted therapies, and immunotherapy, including therapeutic human papillomavirus vaccines, are reviewed.

Summary: Despite recent therapeutic breakthroughs, advanced and/or recurrent endometrial cancer remains a significant health burden globally. Our study suggests that NPM/B23-induced CD24 expression in endometrial cancer cells plays a role in immune evasion by inhibiting macrophage-mediated phagocytosis. CD24 may serve as a novel target for endometrial cancer immunotherapy.

Summary: Heterogeneity assessment using first-order and texture analysis metrics in F-18-fluorodeoxyglucose PET imaging was evaluated for classification of metastatic and benign bone lesions in cervical cancer patients. SUVsd was found to be superior to SUVmax in classification, but careful validation is required for multiple features and parameters in texture analysis.

Development of New Cancer Treatment by Identifying and Focusing the Genetic Mutations or Altered Expression in Gynecologic Cancers

Summary: The Cancer Genome Atlas research network has provided molecular classifications for gynecologic cancers with the use of NGS technology, impacting clinical practice. New cancer treatments targeting pathogenic gene abnormalities, particularly PARP inhibitors, have shown breakthrough benefits in reducing hazard ratios for BRCA1/2 mutated ovarian cancer. Immune checkpoint inhibition also shows promise in cancers with mismatch repair deficiency or microsatellite instability.

A Patient-Derived Xenograft Model of Dedifferentiated Endometrial Carcinoma: A Proof-of-Concept Study for the Identification of New Molecularly Informed Treatment Approaches

Summary: Reliable animal models are crucial for studying novel treatment approaches in human malignancies. A patient-derived xenograft model termed PDX-mLung was developed for dedifferentiated endometrial carcinoma, showing high similarity and druggable genetic alterations. Combination treatment with FGFR inhibitor and cell cycle inhibitor exhibited synergistic therapeutic effects in vivo, highlighting the potential of molecularly informed therapies in difficult-to-treat cancers.

The latest molecular classifications have identified different subtypes, but undifferentiated/dedifferentiated EC has not been mentioned. Summary: Embryo-endometrial communication plays a critical role in successful embryo implantation and pregnancy establishment through the regulation of maternal immune responses. Recent research has highlighted the importance of extracellular vesicles (EVs) and microRNAs in this process, showing that they modulate maternal immune responses and promote cross-communication between the embryo and the endometrium. Exploring the functions and mechanisms of EVs in embryo implantation and pregnancy may provide valuable insights for reproductive medicine and infertility treatments.

East Asian Gynecologic Oncology Trial Group (EAGOT): founding history and future perspective

Summary: There are racial and regional differences in gynecologic cancer, but most large-scale phase III studies have been conducted in Western countries, limiting data on Asians. To address this, four clinical trial groups in East Asia have established the East Asia Gynecologic Oncology Trial Group (EAGOT) to conduct international clinical trials and provide the best treatments for Asian women. EAGOT aims to establish standards of care and guidelines for gynecologic cancer in Asia.
