Summary: This article reviews the pharmacokinetics and safety of subcutaneous testosterone therapy and provides guidance for clinicians on counseling and managing patients who choose this route. Limited evidence suggests that subcutaneous administration results in comparable pharmacokinetics and testosterone levels as intramuscular injection. The subcutaneous route is easier for self-administration and has the potential to improve patient adherence.

Summary: In patients with Late-onset hypogonadism (LOH) and castrated mice, supplementation of testosterone led to upregulation of miR-125a-5p in plasma, which was positively associated with testosterone concentrations. Further analysis revealed LYPLA1 as a target gene of miR-125a-5p, and testosterone deficiency resulted in decreased global protein palmitoylation level. Our findings suggest that testosterone may regulate global palmitoylation level through the miR-125a-5p/LYPLA1 signaling pathway in LOH, potentially contributing to the molecular mechanism underlying LOH symptoms.

Summary: Although testosterone replacement therapy is commonly used for men with classical hypogonadism, its role in age-related decline of testosterone remains unclear due to limited evidence from long-term clinical trials. However, men aged 50 years and above with comorbidities and higher body mass index often exhibit symptoms of androgen deficiency and low testosterone levels. Clinicians face a challenging dilemma in initiating testosterone therapy and must carefully assess the benefits and risks based on limited clinical trial data. In this article, a practical approach to the assessment and management of such men is presented using a case scenario.

Decreased libido, erectile dysfunction, and orgasmic disorder are common manifestations. Depression may cause male functional hypogonadism. Antidepressants can worsen sexual complaints. The interaction between depression and male hypogonadism and the effect of treatment on both conditions are still unclear.

Summary: Late-onset hypogonadism (LOH) is commonly treated with testosterone replacement therapy. This study developed a new 2% testosterone gel (NTG) and compared its effectiveness with the imported testosterone 2% (AndroForte 2 (R) [AF2]) in improving serum testosterone concentrations and Aging Males' Symptoms (AMS) scores in LOH patients. The results showed that NTG significantly increased serum testosterone concentrations and improved AMS scores, while AF2 did not have the same effect. This initial study suggests that the new NTG formulation may be an effective treatment for LOH.

Summary: A multidisciplinary panel from BSSM reviewed the literature on testosterone deficiency (TD) and provided evidence-based statements for clinical practice. Recent studies have shown that low testosterone levels in men are associated with an increased risk of type 2 diabetes, worse outcomes in chronic kidney disease, COVID-19 infection with increased mortality, and significant impacts on quality of life. These guidelines will aid practitioners in diagnosing and managing primary and age-related TD effectively.

Summary: The specific role of testosterone replacement therapy in late onset hypogonadism is still debated. Different preparations have been developed for different purposes, and their advantages and limitations are discussed. The selection of a specific testosterone preparation should be discussed with each patient. Transdermal preparations can maintain the circadian rhythmicity of testosterone secretion, while short-acting preparations may require more frequent administration. Long-acting preparations, such as injectable testosterone undecanoate, offer the advantage of less frequent administration. The use of non-steroidal selective androgen receptor modulators (SARMs) is still investigational, despite their potential use in the treatment of muscle wasting and osteoporosis.

Summary: This study compared the safety and efficacy of compounded and market brand testosterone pellets for testosterone therapy in men with testosterone deficiency. The results showed similar serum testosterone levels and adverse event rates between the two groups. Therefore, compounded testosterone pellets are a noninferior option for testosterone therapy in men with testosterone deficiency when compared to market brand pellets.

Summary: The evidence for age-related effects on wound healing is mostly based on empirical observations without adjusting for confounding factors. Skin structure changes with aging, but the impact on unexposed skin seems to be minimal. The clinical impact of these changes on acute wound healing appears to be small compared to other factors. Poor healing of chronic wounds, mainly observed in older populations, is often attributed to comorbid conditions rather than age alone.
