Summary: Androgen prohormones, such as DHEA, increase in early puberty, peak in the second and third decade, and decline thereafter. Studies have examined the potential benefits of DHEA therapy in various hormone-deficient states and in normal women. The data support small benefits in quality of life and mood for women with hormone deficiencies, but not for anxiety or sexual function. However, no consistent beneficial effects have been observed in menopausal symptoms, sexual function, cognition, or overall wellbeing in normal women. Local administration of DHEA may be beneficial for vulvovaginal atrophy, but caution is advised for long-term use due to the risks of androgenic and estrogenic side effects.

Summary: This study reported a case of a testosterone deficient man converting from CC to Natesto, showing normalization of gonadotropins and a rise in semen parameter values after dose titration. The rest of the men also demonstrated stability in gonadotropins and semen parameters after the conversion. Dose titration of Natesto may be effective at optimizing hormonal levels, semen parameters, testosterone levels, and symptomatic response.

Summary: This study reports the prevalence of male testosterone deficiency among long-term male survivors of malignant lymphoma. It shows that for screening purposes, an initial measurement of total testosterone may be sufficient in some survivors, but subnormal concentrations of calculated free testosterone were found in 15% of the survivors. Intense treatment for Hodgkin lymphoma and older age were identified as high-risk factors for testosterone deficiency. Pituitary downregulation was also observed. Overall, longitudinal measurements of total testosterone alone may not be adequate for screening testosterone deficiency in all long-term lymphoma survivors.

Summary: This study aimed to evaluate the relationship between gonadal hormone concentrations in blood and frailty syndrome, as well as whether these hormone concentrations could predict the progression of frailty syndrome. The results showed that high androstenedione levels and low follicle-stimulating hormone (FSH) levels were significantly associated with the worsening of frailty syndrome. These hormone concentrations could be potential biomarkers to predict the progression of frailty syndrome.

Summary: This commentary evaluates the use of pharmacological treatment for prediabetes. The author concludes that treating the dysglycemia of prediabetes with medication is unnecessary, but considering the ability of high doses of GLP-1 receptor agonists and the combination of GLP-1 receptor agonist and GIP to lower weight, it deserves consideration for prediabetes treatment. This amount of weight loss should decrease insulin resistance and lower the risk of developing diabetes.

Summary: This study found that spinal canal compromise and kyphotic angle were associated with nonoperative treatment failure in TLICS 4 fracture patients. Other factors such as age, sex, BMI, initial VAS score, and loss of vertebral body height were not significantly related to treatment failure. Therefore, it is recommended that surgeons pay more attention to spinal canal compromise and kyphotic angle in deciding the treatment for TLICS 4 fracture patients.

Summary: Testosterone levels are correlated with sarcopenia and hypogonadism in patients with liver cirrhosis. Exogenous testosterone supplementation can improve body composition, but has limited impact on patient survival. Low testosterone levels are associated with decreased muscle mass and increased frailty in patients with chronic liver disease, leading to higher morbidity and mortality. Exogenous testosterone supplementation improves anthropometric parameters, but does not significantly affect morbidity or mortality in patients, and its long-term safety needs further investigation.

Summary: Knee joint osteoarthritis is a complex immunological and degenerative disease, and current treatment strategies are ineffective in altering its progression. Mesenchymal stromal cell (MSC) therapy, which aims to regenerate the affected knee joint structures, has been researched for over 30 years. The secretome of MSCs has been found to play a central role in their regenerative capacity. Animal models have demonstrated the symptom- and disease-modifying effects of MSC therapy. Clinical studies have shown significant improvement in symptoms and functional outcomes with autologous and allogeneic MSC therapy, although contradictory results exist in the literature. The effectiveness of MSC therapy is still unclear and further research is needed.
