Dipeptidyl peptidase 4 (DPP4, CD26) is a serine protease that is expressed constitutively by many haematopoietic and non-haematopoietic tissues. It exists as a membrane-associated protein, as well as in an active, soluble form (herein called sDPP4), present at high concentrations in bodily fluids. Despite the proposed use of sDPP4 as a biomarker for multiple diseases, its cellular sources are not well defined. Here, we report that individuals with congenital lymphocyte immunodeficiency had markedly lower serum concentrations of sDPP4, which were restored upon successful treatment and restoration of lymphocyte haematopoiesis. Using irradiated lymphopenic mice and wild-type to Dpp4(-/-) reciprocal bone marrow chimeric animals, we found that haematopoietic cells were a major source of circulating sDPP4. Furthermore, activation of human and mouse T lymphocytes resulted in increased sDPP4, providing a mechanistic link between immune system activation and sDPP4 concentration. Finally, we observed that acute viral infection induced a transient increase in sDPP4, which correlated with the expansion of antigen-specific CD8(+) T cell responses. Our study demonstrates that sDPP4 concentrations are determined by the frequency and activation state of lymphocyte populations. Insights from these studies will support the use of sDPP4 concentration as a biomarker for inflammatory and infectious diseases.

Summary: Common variable immunodeficiency disorders (CVID) is a primary immune deficiency characterized by hypogammaglobulinemia and impaired antibody responses. Intravenous Immunoglobulin (IVIg), a preparation of highly purified poly-specific IgG, exerts immunoregulatory and anti-inflammatory actions on peripheral blood monocytes, leading to an acquisition of an anti-inflammatory gene profile and altered proportions of monocyte subsets. CD14+ MDSC-like cells may contribute to the immunoregulatory effects of IVIg in CVID and other inflammatory disorders.

Summary: This study describes a patient with tubular aggregate myopathy and Stormorken syndrome (TAM/STRMK) caused by a novel STIM1 mutation. In addition to typical TAM/STRMK manifestations, the patient also exhibited immunological involvement with respiratory infections, chronic cough, and chronic bronchiectasis. Dysregulation of calcium signaling in immune cells may contribute to the immune involvement observed in this patient.

Summary: This study aimed to evaluate the role of monocyte subsets and cytokines in RPL and RIF women. The study found that CX3CR1+ and CCR5+ intermediate monocytes were significantly higher in RPL and RIF, and there was a significant positive correlation between CX3CR1+ intermediate monocytes and IL-17A. The combination of specific monocyte subsets and cytokines could aid in identifying RPL and RIF women with a pro-inflammatory profile.

Summary: Through a genome-wide rare variant burden association analysis, it was found that there is an association between at-risk variants in the TLR7 gene and rare loss-of-function variants in TLR3-dependent type I interferon immunity genes. These findings suggest that rare variants in TLR3- and TLR7-dependent type I interferon immunity genes may underlie life-threatening COVID-19 in patients under 60 years old.

Summary: This study retrospectively investigated the global incidence of autoantibodies associated with myositis (non Jo1-RNA synthetases: antiPL7, anti-PL12, anti-EJ, anti-OJ and RNA-sensor: anti-MDA5) in a cohort of 444 patients with suspected inflammatory myopathy from 2015 to 2021. The results showed an increased incidence of anti-synthetase antibodies during the COVID-19 pandemic and SARS-CoV-2 vaccination, with differential profiles of antibody specificities observed between 2020 and 2021.
