Remote Ischaemic Conditioning in STEMI Patients in Sub-Saharan AFRICA: Rationale and Study Design for the RIC-AFRICA Trial

Kishal Lukhna, Derek J. Hausenloy, Abdelbagi Sidahmed Ali, Abdullah Bajaber, Alistair Calver, Arthur Mutyaba, Awad Abdalla Mohamed, Brian Kiggundu, Chishala Chishala, Ebrahim Variava, Ehab Ali Elmakki, Elijah Ogola, Eltayeb Hamid, Emmy Okello, Isam Gaafar, Keiran Mwazo, Makoali Makotoko, Mergan Naidoo, Mohamed Elhadi Abdelhameed, Motasim Badri, Nasief van der Schyff, Omaima Abozaid, Paul Xafis, Sara Giesz, Trevor Gould, Waldo Welgemoed, Malcolm Walker, Mpiko Ntsekhe, Derek M. Yellon

Summary: Despite evidence of myocardial infarct size reduction in animal studies, remote ischaemic conditioning (RIC) failed to improve clinical outcomes in the large CONDI-2/ERIC-PPCI trial. The RIC-AFRICA trial aims to investigate whether RIC can improve clinical outcomes in higher-risk STEMI patients in environments with limited access to early reperfusion or PPCI.

Cellular interplay between cardiomyocytes and non-myocytes in diabetic cardiomyopathy

Ren Jie Phang, Rebecca H. Ritchie, Derek J. Hausenloy, Jarmon G. Lees, Shiang Y. Lim

Summary: Patients with Type 2 diabetes often suffer from a distinct cardiac condition called diabetic cardiomyopathy, which is not well understood and has limited treatment options. This review focuses on the roles of non-myocyte cells in the heart and the potential use of sodium-glucose cotransporter 2 inhibitors as a therapy for diabetic cardiomyopathy.

Summary: This article provides an overview of the techniques and technologies available for the separation and characterization of EVs from different sources. It discusses methods for determining the protein, RNA, and lipid content of EVs. The guidance provided in this document is important for investigating the role of EVs in cardiovascular diseases and highlights key methodological issues to consider.

A distribution-free smoothed combination method to improve discrimination accuracy in multi-category classification

Raju Maiti, Jialiang Li, Priyam Das, Xueqing Liu, Lei Feng, Derek J. Hausenloy, Bibhas Chakraborty

Summary: In this article, a linear combination method is proposed to improve the overall diagnostic accuracy by combining results from multiple diagnostic tests. The method involves a smooth approximation of the empirical Hyper-volume Under Manifolds and employs efficient gradient-based algorithms for better solutions. Simulation study and application to real medical datasets demonstrate the effectiveness of the proposed method.

Effects of fitting methods, high b-values and image quality on diffusion and perfusion quantification and reproducibility in the calf

Ying-Hwey Nai, Xiaomeng Wang, Julian Gan, Cheryl Pei Ling Lian, Ryan Fraser Kirwan, Forest Su Lim Tan, Derek J. Hausenloy

Summary: The study aimed to optimize DWI image acquisition and analysis protocols in calf muscles by investigating the effects of different model-fitting methods, image quality, and use of high b-value and constraints on POIs. The optimized models showed that ADC and Dd were reliable for calf diffusion evaluation, with CVs of 7.22% and 4.09%.

Unravelling the Interplay between Cardiac Metabolism and Heart Regeneration

Fan Yu, Shuo Cong, En Ping Yap, Derek J. Hausenloy, Chrishan J. Ramachandra

Summary: Ischemic heart disease is a major cause of heart failure and has a significant impact on global health. The limited regenerative capacity of cardiomyocytes poses challenges for heart repair. Recent research has shown that cardiac metabolism plays a crucial role in heart regeneration and modulating this metabolism could promote regeneration. However, there is a lack of mechanistic understanding in this field, hindering the development of effective therapeutic strategies.

Summary: The incidence of AMI is projected to continue rising, with overweight/obesity emerging as the fastest-growing metabolic risk factor and the leading risk factor for AMI-related mortality. Individuals of Indian and Malay descent bear a disproportionate burden of overweight/obesity incidence and AMI-related mortality.

Health position paper and redox perspectives on reactive oxygen species as signals and targets of cardioprotection

Hausenloy, Borja Ibanez, Thomas Krieg, Christoph Maack, Rainer Schulz, Frank Sellke, Ajay M. Shah, Holger Thiele, Derek M. Yellon, Fabio Di Lisa

Summary: This review summarizes the roles of reactive oxygen species (ROS) in myocardial ischemia/reperfusion injury and cardioprotection. It emphasizes the need for cardioprotection beyond rapid reperfusion in acute myocardial infarction and discusses the pathomechanisms of myocardial ischemia/reperfusion, different modes of cell death in myocardial infarction, and interventions to protect the ischemic/reperfused myocardium. The second part focuses on ROS, providing an overview of molecular and cellular mechanisms involved in ischemia/reperfusion injury.

Defining Peri-Operative Myocardial Injury during Cardiac Surgery Using High-Sensitivity Troponin T

Vikram Sharma, Huili Zheng, Luciano Candilio, Jennifer M. Nicholas, Tim Clayton, Derek M. Yellon, Heerajnarain Bulluck, Derek J. Hausenloy

Summary: Baseline and post-operative elevations in high-sensitivity troponin T are independently associated with 1-year all-cause mortality in cardiac surgery patients. An elevation in hs-TnT ≥50 x URL at 24 hours post-surgery is the optimal threshold to define prognostically significant peri-operative myocardial injury.

Importance of Mitochondria in Cardiac Pathologies: Focus on Uncoupling Proteins and Monoamine Oxidases

Rainer Schulz, Klaus-Dieter Schlueter

Summary: Reactive oxygen species (ROS) play dual roles in disease development and cell signaling pathways. Cardiomyocytes generate the largest amount of ROS through mitochondria. Uncoupling protein (UCP) and monoamine oxidases (MAO) have been identified as modifiers of mitochondrial ROS formation. This review highlights the role of UCP and MAO in cardiac injuries induced by ischemia-reperfusion (I/R) and their potential for protection from I/R and heart failure.

Sauri Hernandez-Resendiz, Aishwarya Prakash, Sze Jie Loo, Martina Semenzato, Kroekkiat Chinda, Gustavo E. Crespo-Avilan, Linh Chi Dam, Shengjie Lu, Luca Scorrano, Derek J. Hausenloy

Summary: Acute myocardial ischemia-reperfusion injury is a crucial factor in myocardial infarction and heart failure. Researchers have found that therapeutic strategies aimed at preserving mitochondrial shape and function may help protect the heart and improve health outcomes in patients with acute myocardial infarction.
