Optimal Avapritinib Treatment Strategies for Patients with Metastatic or Unresectable Gastrointestinal Stromal Tumors

Cissimol P. Joseph, Sarah N. Abaricia, Michelle A. Angelis, Kathleen Polson, Robin L. Jones, Yoon-Koo Kang, Richard F. Riedel, Patrick Schoffski, Cesar Serrano, Jonathan Trent, Eric D. Tetzlaff, Tuan Dong Si, Teresa Zhou, Ashley Doyle, Sebastian Bauer, Maria Roche, Tracy Havnaer

Summary: Avapritinib, a novel inhibitor approved for U/M GISTs, showed common treatment-related AEs including nausea, fatigue, and anemia. Cognitive effects were observed in a significant percentage of patients, particularly in older patients, but improved faster with dose modification. Utilizing tolerability-guided dose modification may be an effective strategy in managing AEs and maintaining patients on avapritinib treatment.

Ripretinib intrapatient dose escalation after disease progression provides clinically meaningful outcomes in advanced gastrointestinal stromal tumour

Suzanne George, Ping Chi, Michael C. Heinrich, Margaret von Mehren, Robin L. Jones, Kristen Ganjoo, Jonathan Trent, Hans Gelderblom, Albiruni A. Razak, Michael S. Gordon, Neeta Somaiah, Julia Jennings, Julie Meade, Kelvin Shi, Ying Su, Rodrigo Ruiz-Soto, Filip Janku

Summary: The study demonstrated that Ripretinib intrapatient dose escalation (IPDE) in GIST patients can provide continued clinical benefit across second, third, and later lines of therapy. Comparison of treatment-emergent adverse events (TEAEs) at different doses showed similar safety profiles.

Clinical Outcomes for Primary and Radiation-Associated Angiosarcoma of the Breast with Multimodal Treatment: Long-Term Survival Is Achievable

Joshua P. Kronenfeld, Jessica S. Crystal, Emily L. Ryon, Sina Yadegarynia, Celeste Chitters, Raphael Yechieli, Gina D'Amato, Andrew E. Rosenberg, Susan B. Kesmodel, Jonathan C. Trent, Neha Goel

Summary: Primary angiosarcoma of the breast (PAS) and radiation-associated angiosarcoma of the breast (RAAS) are rare sarcomas with incidences of 0.05% and 0.02%, respectively. Multimodal treatment with neoadjuvant chemotherapy, radiation therapy, surgery, and adjuvant chemotherapy can result in optimal clinical outcomes. However, further prospective, multi-institutional studies are needed to overcome the limitations associated with the low incidence of breast angiosarcoma.

Immunotherapy Strategies for Gastrointestinal Stromal Tumor

Junaid Arshad, Philippos A. Costa, Priscila Barreto-Coelho, Brianna Nicole Valdes, Jonathan C. Trent

Summary: Gastrointestinal stromal tumors (GIST) are the most common type of gastrointestinal sarcomas, and recent treatment advancements have significantly improved patient survival rates. However, metastatic GIST patients may require multiple lines of treatment, and the potential role of immunotherapies in enhancing current therapies needs further exploration through in-depth research.

Diaz-Perez, William M. Korn, Matthew J. Oberley, Daniel Magee, Don Dizon, Margaret von Mehren, Moh'd M. Khushman, Atif Mahmoud Hussein, Kirsten Leu, Jonathan C. Trent

Summary: Angiosarcomas are rare and highly aggressive sarcomas with limited treatment options. Genomic sequencing has identified recurrent genetic abnormalities in different primary sites of AS, which can help in designing and optimizing therapeutic strategies. The study analyzed NGS and WTS data in a cohort of 143 cases, revealing distinct genomic biology according to the AS primary site.

Antibiotic Stewardship and Inpatient Clostridioides difficile Testing in Solid Organ Transplant Recipients: The Need for Multilevel Checks and Balances

Michael Kueht, Ahmad Kharsa, Muhammad Mujtaba, Rupak Kulkarni, Rachel Britt, A. Scott Lea, Ghani Hafiz, Jeffery Fair, David Reynoso

Summary: This study aims to demonstrate the association of an institutional integrated stewardship program with C. difficile testing practices after abdominal solid organ transplantation. The results show that the program significantly decreased the number of C. difficile tests, reduced the cost of inpatient treatment and days of therapy, without affecting the rate of negative testing.

Prediction of the need for intensive oxygen supplementation during hospitalisation among subjects with COVID-19 admitted to an academic health system in Texas: a retrospective cohort study and multivariable regression model

John W. Davis, Beilin Wang, Ewa Tomczak, Chia Chi-Fu, Wissam Harmouch, David Reynoso, Philip Keiser, Miguel Mauricio Cabada

Summary: The study aimed to develop a model to predict the likelihood of COVID-19 patients progressing to severe hypoxemic respiratory failure using demographic and clinical data collected upon admission. The results showed that lactate dehydrogenase, Quick Sequential Organ Failure Assessment score, and neutrophil to lymphocyte ratio were significant predictors of severe disease. The model demonstrated high accuracy in predicting the deterioration of COVID-19.

Severity of COVID-19 in Cancer patients versus patients without Cancer: A Propensity Score Matching Analysis

Chao Liu, Kai Wang, Luyuan Li, Qingquan Lv, Yumei Liu, Tian Hu, Jonathan C. Trent, Bing Sun, Qinyong Hu

Summary: The study found distinct clinical features in COVID-19 patients with cancer compared to those without cancer, including leukopenia, elevated hs-CRP, and procalcitonin, as well as poorer outcomes in cancer patients with critical COVID-19. Recent cancer diagnosis and anti-tumor treatment within 3 months of COVID-19 diagnosis were associated with inferior survival in the cancer cohort.
