Design and evaluation of a treatment programme for Spanish adolescents with overweight and obesity. The EVASYON Study

Background: The prevalence of overweight and obesity (OW/OB) among adolescents worldwide has increased since the 60 s. Spain has reached one of the highest OW/OB prevalence rates among adolescents from European countries. The aim of this methodological paper is to describe the design and evaluation in the EVASYON study (Development, implementation and evaluation of the efficacy of a therapeutic programme for adolescents with OW/OB: integral education on nutrition and physical activity). Methods/Design: The EVASYON was planned by a multidisciplinary team to treat OW/OB in Spanish adolescents. The EVASYON is a multi-centre study conducted in 5 hospitals in 5 Spanish cities (Granada, Madrid, Pamplona, Santander and Zaragoza) and two hundred and four OW/OB Spanish adolescents were recruited for this intervention. The treatment was implemented for approximately one-year follow-up. The adolescents were treated in groups of a maximum of 10 subjects; each group had 20 visits during the treatment period in two phases: intensive during the first 2 months (1(st) to 9(th) visits), and extensive during the last 11 months (10(th) to 20(th) visits). In order to assess the efficacy of the treatment, 8 dimensions were measured: diet; physical activity and fitness; eating behaviour; body composition; haematological profile; metabolic profile; minerals and vitamins; immuno-inflammatory markers. Moreover, genetic polymorphisms were also determined. Discussion: The treatment programme developed in the EVASYON study was designed as a national pilot study to be implemented as an effective treatment for adolescents with OW/OB into the Spanish Health Care Service.

Summary: This study found that higher levels of total and prolonged sedentary time are associated with greater depressive symptoms and state anxiety in women with fibromyalgia. Moderate-to-vigorous physical activity did not modify these associations, although physical fitness could play a protective role specially for state anxiety.

Association of urinary and ambient black carbon, and other ambient air pollutants with risk of prediabetes and metabolic syndrome in children and adolescents

Summary: This study investigated the association between urinary black carbon (BC) particles and ambient BC and the risk of prediabetes and metabolic syndrome (MetS) in children. The results showed a positive association between urinary BC particles and MetS, as well as an association between ambient BC and MetS in children living closer to a major road. These findings suggest that exposure to BC may contribute to the risk of MetS in children.

New indices in predicting cardiometabolic risk and its relation to endothelial dysfunction in adolescents: The HELENA study

Summary: This study aimed to evaluate the predictive power of TyG, TG/HDL-c, HLAP, and VAI indices in identifying cardiometabolic risk factors and their relationship with endothelial dysfunction biomarkers in European adolescents. The results showed that TG/HDL-c and HLAP indices performed well in predicting cardiometabolic risk in male adolescents, but their association with endothelial dysfunction caused by insulin resistance was not clear.

The results showed that neither HbA1c nor fasting glucose can predict short-term weight loss success, but they may impact the metabolic response to rapid weight loss.

Clinical and physical characteristics of thinness in adolescents: the HELENA study

Summary: The study aimed to assess the prevalence, characteristics, and health impacts of thinness in European adolescents. The results showed that thin adolescents had lower blood pressure, later onset of first menstrual cycle in girls, lower upper-body muscular strength, and shorter time spent in light physical activity. However, thinness did not have significant adverse health consequences on European adolescents.

Body image and dietary habits in adolescents: a systematic review

Summary: This systematic review found associations between body image perception and satisfaction and dietary habits in adolescents. Accurate body image perception was related to healthy diets, while underestimation of body weight was associated with unhealthy diets. Adolescents who were unsatisfied with their body image and desired thinness engaged in diets linked to weight loss.

Ketogenic diet in children and adolescents: The effects on growth and nutritional status

Summary: The ketogenic diet has been increasingly used for weight loss among adolescents and young adults due to its supposed effects on lipolysis and insulin levels. However, there is still a lack of clear indications on its use for weight loss in pediatric age. It has also been proposed for other disorders like inherited metabolic disorders, Prader-Willi syndrome, and certain types of cancers. A clinical evaluation of possible side effects, growth, and nutritional status is essential for patients following a long-term restrictive diet like the ketogenic one.

Summary: There is limited evidence on the early nutrition programming of glucose homeostasis in children. This study found that breastfeeding and normal/slow weight gain in the first 6 months of life appear to protect against dysregulation of glucose homeostasis at 6 years old. The experimental formula showed functional similarities to breastfeeding in terms of children's glucose variability.

The ALINFA Intervention Improves Diet Quality and Nutritional Status in Children 6 to 12 Years Old

Summary: This study aimed to evaluate the effectiveness of a new nutritional intervention in improving the quality of diet among children aged 6 to 12 years. The intervention group received a normocaloric diet with specially designed products and recipes, while the control group received standard advice on healthy eating. The intervention group showed improvements in diet quality, reduced calorie and fat intake, increased fiber intake, and decreased BMI and waist circumference.

It will enroll 2296 healthy term-born infants and randomize them into two groups to be fed with either goat milk formula or cow milk formula. The study will observe the cumulative incidence of atopic dermatitis and other indicators within 12 months.

Maternal weight, gut microbiota, and the association with early childhood behavior: the PREOBE follow-up study

Summary: It is found that maternal overweight, obesity, and gestational diabetes during pregnancy may affect the behavior of children. The study analyzed the behavior of children at 3.5 years old and the gut microbiota established at 18 months old. The results showed that children born to overweight/obese mothers had higher scores in behavioral problems, and those born to obese mothers with gestational diabetes had even higher scores in attention/deficit hyperactivity and externalizing problems. The composition of the gut microbiota was also associated with behavioral problems in children.
