The process of somatic hypermutation increases polyreactivity for central nervous system antigens in primary central nervous system lymphoma

Summary: The mutational pattern of the B-cell receptor in PCNSL cells suggests antigenic selection and confinement in the CNS, supporting the pathogenesis of the disease. Abnormal GC reaction in PCNSL may increase self-/polyreactivity, facilitating BCR signaling via multiple CNS antigens and promoting tumor cell survival in the CNS.

Impact of a Faulty Germinal Center Reaction on the Pathogenesis of Primary Diffuse Large B Cell Lymphoma of the Central Nervous System

Manuel Montesinos-Rongen, Anna Brunn, Monica Sanchez-Ruiz, Ralf Kueppers, Reiner Siebert, Martina Deckert

Summary: The pathogenesis of primary CNS lymphoma (PCNSL) involves a failure in B cell differentiation and a lack of control during the entrance and within the germinal center (GC) stages. Targeted molecular studies have shown that activated pathways promote lymphoma cell proliferation, leading to tumor cells being trapped in a late B cell differentiation stage. This may contribute to the increased affinity of the lymphoma for the CNS.

Schmitt, Hans Christian Reinhardt, Gero Knittel

Summary: This study successfully recapitulated the characteristics of human ABC-DLBCL in a mouse model, indicating a cooperative role of Myd88 and BCL2 in lymphomagenesis and demonstrating substantial preclinical antilymphoma activity with combined BCL2 and PD-1 blockade.
