Expression of Transient Receptor Potential C6 Channels in Human Lung Macrophages

Chronic obstructive pulmonary disease (COPD) is associated with pulmonary inflammation with increased numbers of macrophages located in the parenchyma. These macrophages have the capacity to mediate the underlying pathophysiology of COPD; therefore, a better understanding of their function in chronic inflammation associated with this disease is vital. Ion channels regulate many cellular functions; however, their role in macrophages is unclear. This study examined the expression and function of transient receptor potential (TRP) channels in human macrophages. Human alveolar macrophages and lung tissue macrophages expressed increased mRNA and protein for TRPC6 when compared with monocytes and monocyte-derived macrophages. Moreover, TRPC6 mRNA expression was significantly elevated in alveolar macrophages from patients with COPD compared with control subjects. There were no differences in mRNA for TRPC3 or TRPC7. Although mRNA for TRPM2 and TRPV1 was detected in these cells, protein expression could not be determined. Fractionation of lung-derived macrophages demonstrated that TRPC6 protein was more highly expressed by smaller macrophages compared with larger macrophages. Using whole-cell patch clamp electrophysiology, TRPC6-like currents were measured in both macrophage subpopulations with appropriate biophysical and basic pharmacological profiles. These currents were active under basal conditions in the small macrophages. These data suggest that TRPC6-like channels are functional on human lung macrophages, and may be associated with COPD.

Summary: Optimizing asthma diagnosis is crucial for reducing the burden of asthma globally. A compound diagnostic approach and label are now required due to the complexity and heterogeneity of different forms of asthma. By recognizing typical symptoms and abnormal physiology, asthma can be identified, and further features such as eosinophilic or type 2 inflammation can be used to give a compound diagnostic label of eosinophilic asthma, which provides important information about future exacerbation risk and treatment responsiveness. Implementing treatable traits and using targeted assessment allows for a specific treatment plan based on a pragmatic and iterative process. Anti-Tumor Necrosis Factor for Supplementary Management in Severe Asthma: A Systematic Review and Meta-analysis

Summary: This study evaluated the efficacy and safety of anti-tumor necrosis factor (TNF) as a supplementary therapy for patients with severe asthma. The results showed that anti-TNF treatment improved asthma control, but there was limited evidence for improvement in lung function and reduction of asthma exacerbation. Therefore, prescribing anti-TNF in adults with severe asthma is unlikely.

Single cell quantification of microRNA from small numbers of non-invasively sampled primary human cells

Summary: An amplification-free sandwich hybridisation assay using a microfluidic platform is developed to detect single miRNA molecules in single cells. The absolute quantification of miRNAs at a single cell level is achieved and validated using real-time qPCR. The sensitivity of the assay is demonstrated by quantifying single miRNA molecules in nasal fluid, showing its potential utility in clinical studies.

Summary: This study aimed to explore perspectives on spirometry provision in primary care and the potential for artificial intelligence (AI) decision support software to improve quality and interpretation. The study found historical challenges and the impact of the pandemic on inequity in spirometry provision. It suggests delivering services closer to patients’ homes by appropriately trained staff and highlights the potential role of AI in supporting clinicians. However, sufficient validation of the software is crucial for clinician and healthcare commissioner trust in the process.

Rational use of inhaled corticosteroids for the treatment of COPD

Summary: Inhaled corticosteroids (ICS) are commonly used for asthma treatment, but their role in chronic obstructive pulmonary disease (COPD) is still a topic of debate. Recent trials have shown that ICS combined with dual bronchodilator therapy can significantly reduce exacerbations in COPD patients. However, the suggestion of a mortality benefit associated with ICS in these trials has been rejected. Observational evidence suggests that dual bronchodilation is associated with better outcomes than triple therapy in a broad population of COPD patients. It is recommended that ICS be reserved for patients with frequent or severe exacerbations and high blood eosinophils or those with concomitant asthma.
