Presentation, Diagnosis, and Management of Subglottic and Tracheal Stenosis During Systemic Inflammatory Diseases

Jennifer Catano, Yurdagul Uzunhan, Romain Paule, Jeremie Dion, Alexis Regent, Paul Legendre, Francois Gonin, Emmanuel Martinod, Pascal Cohen, Xavier Puechal, Veronique Le Guern, Luc Mouthon, Andre Coste, Christine Lorut, Candice La Croix, Sophie Perie, Benjamin Terrier

Summary: This study found that GPA and RP are the two main inflammatory diseases presenting with SGS/TS. GPA-related stenoses are mostly subglottic and circumferential, whereas RP-related stenoses are mostly tracheal, anterior, and calcified with a frequent extension to bronchi.

Treatment of Idiopathic Pulmonary Fibrosis with Capsule or Tablet Formulations of Pirfenidone in the Real-Life French RaDiCo-ILD Cohort

Vincent Cottin, Sonia Gueguen, Hilario Nunes, Stephane Jouneau, Bruno Crestani, Philippe Bonniaud, Lidwine Wemeau, Dominique Israel-Biet, Martine Reynaud-Gaubert, Anne Gondouin, Jacques Cadranel, Sylvain Marchand-Adam, Marie Chevereau, Isabelle Dufaure-Gare, Serge Amselem, Annick Clement

Summary: This study in France on IPF patients using pirfenidone capsules and tablets showed good acceptability of the tablet formulation among patients, suggesting possible advantages over the capsules. The study highlighted no new adverse events and lower discontinuation rates in the tablet and substitution subgroups compared to the capsules-only subgroup.

Summary: This study describes the phenotype of patients with interstitial lung disease (ILD) and heterozygous PARN mutations, showing that idiopathic pulmonary fibrosis is common among individuals with PARN mutation. Other ILD subtypes may also be observed, and some patients may experience disease progression leading to death or lung transplantation despite specific treatment.

Estimation of Regional Pulmonary Compliance in Idiopathic Pulmonary Fibrosis Based on Personalized Lung Poromechanical Modeling

Cecile Patte, Pierre-Yves Brillet, Catalin Fetita, Jean-Francois Bernaudin, Thomas Gille, Hilario Nunes, Dominique Chapelle, Martin Genet

Summary: Pulmonary function is closely related to lung mechanical behavior, and idiopathic pulmonary fibrosis (IPF) affects lung mechanics, thus altering lung function. Currently, the mechanical impact of IPF is only assessed through pressure-volume curves which provide global information. We have developed a personalized poromechanical model of the lung that can diagnose IPF based on clinical data.

Summary: This pragmatic study aimed to describe the clinical follow-up and respiratory recovery trajectories of survivors of severe-to-critical COVID-19, as well as identify factors influencing their health-related quality of life. The results showed that although pulmonary function and radiological abnormalities improved up to 1 year post-acute COVID-19, a high percentage of severe-to-critical disease survivors had significant lung sequelae and residual symptoms, warranting prolonged follow-up.

Summary: Through a genome-wide rare variant burden association analysis, it was found that there is an association between at-risk variants in the TLR7 gene and rare loss-of-function variants in TLR3-dependent type I interferon immunity genes. These findings suggest that rare variants in TLR3- and TLR7-dependent type I interferon immunity genes may underlie life-threatening COVID-19 in patients under 60 years old.
