Intratumoral CD8+ Lymphocyte Infiltration as a Prognostic Factor and Its Relationship With Cyclooxygenase 2 Expression and Microsatellite Instability in Endometrial Cancer

Objective Microsatellite instability (MSI) is caused by a defective DNA mismatch repair system. Colorectal cancer in MSI-positive patients is characterized by an increased number of tumor-infiltrating lymphocytes. On the other hand, it has recently been reported that cyclooxygenase 2 (COX-2) suppresses antitumor immunity. The objectives of the present study were to clarify the relationships among MSI status, COX-2 expression, and antitumor immune status and to verify impact of these factors on the prognosis of endometrial cancer. Methods The data of 123 patients with endometrial cancer were analyzed. The numbers of tumor-infiltrating CD8(+) T lymphocytes within cancer cell nests (TILs), as a representative of the antitumor immunity, and COX-2 expression levels in the tumor cells were analyzed by immunohistochemical staining. Microsatellite instability was evaluated by polymerase chain reaction analysis for 11 markers. Fisher exact probability test, Kaplan-Meier method, and proportional hazards analysis were used for the statistical analyses. Results The MSI-positive tumors showed significantly higher grades (G2 or G3) and significantly larger numbers of TILs than did the MSI-negative tumors. The COX-2-high group showed significantly fewer TILs than did the COX-2-low group. Multivariate analysis identified a low number of TILs (<10), positive lymph node involvement, and high tumor malignancy grade as factors independently associated with poor prognosis. The prognosis was significantly poorer in the patients with MSI-positive tumors with high COX-2 expression than in those with MSI-positive tumors showing low COX-2 expression. Conclusions The number of TILs, which was increased by MSI and decreased by COX-2 expression, was associated with a poorer prognosis in patients with endometrial cancer. We also propose that COX-2 may block MSI-activated TILs in the tumor microenvironment.

A retrospective study for investigating the outcomes of endometrial cancer treated with radiotherapy

Summary: Patients with endometrial cancer who received radiotherapy had a significantly poorer prognosis compared to those who underwent surgery, with age, advanced stage, histological type, lymphadenectomy, and adjuvant radiotherapy being independent prognostic factors. Significant differences in survival rates were observed among patients receiving adjuvant therapy, chemotherapy, and no adjuvant therapy for stage I or II, and adjuvant chemotherapy showed significant differences compared to no adjuvant therapy and adjuvant radiotherapy for stage III or IV patients.

Adenocarcinoma in situ or early-stage cervical cancer is a risk factor for preterm delivery after cervical conization: a multicenter observational study

Summary: This study investigated the risk factors associated with preterm delivery after conization. The results showed a higher incidence of nulliparity and a history of preterm delivery in the preterm delivery group. Additionally, the proportion of women with adenocarcinoma in situ (AIS) and cervical cancer was higher in the preterm delivery group. Multiple regression models revealed that cervical cancer or AIS and a short cervix in the second trimester increased the risk of preterm delivery in nulliparous women. Similarly, a history of preterm delivery, cervical cancer or AIS, and a short cervix in the second trimester increased the risk of preterm delivery in primiparous women.

Evaluation of the role of intraoperative frozen section and magnetic resonance imaging in endometrial cancer

Impact of adjuvant chemotherapy on the overall survival of patients with resectable bulky small cell neuroendocrine cervical cancer: a JSGO-JSOG joint study

Summary: This study reviewed the clinicopathological characteristics of small cell neuroendocrine cervical cancer (SCNEC) and identified the optimal treatment. The rates of lymph-node and distant metastasis were significantly higher in T1b2 cases than T1b1 cases. For IB2 and T1bN1M0 SCNEC, the 5-year survival rate with surgery followed by chemotherapy was significantly higher than that with surgery followed by radiation therapy/concurrent chemoradiation therapy.

Evaluation of CD4+cells infiltration as a prognostic factor in cervical intraepithelial neoplasia 2

Summary: The study aimed to identify predictors for the prognosis of CIN2 lesions and assess the prognostic value of the local immune response. Immunohistochemical analysis was conducted to determine the presence of p16-, MCM2-, or APOBEC3G-positive cells. Tumor-infiltrating lymphocytes (TILs) were scored using an automated system. The progression and regression rates of CIN3 were estimated, and a case-control study was performed to screen prognostic factors. CD4+ TIL infiltration was found to be related to CIN2 regression, suggesting its potential use for triaging patients.

Trends and characteristics of fertility-sparing treatment for atypical endometrial hyperplasia and endometrial cancer in Japan: a survey by the Gynecologic Oncology Committee of the Japan Society of Obstetrics and Gynecology

Summary: The objective of this study was to examine the current trends in fertility-sparing treatment for young patients with atypical endometrial hyperplasia and endometrial cancer in Japan. A nationwide, retrospective survey was conducted, and data from 413 patients were collected. The study found that the most common treatments used were medroxyprogesterone alone or in combination with metformin, and the success rate of initial treatment was 78.2%. Factors such as histology, body mass index, treatment period, time to complete remission, maintenance therapy, and pregnancy had an impact on recurrence risk and pregnancy outcomes.

Clinical availability and characteristics of multigene panel testing for recurrent/advanced gynecologic cancer

Summary: This study aimed to determine the potential availability and utility of gene panel testing clinically in gynecologic oncology. The study found that many patients had druggable gene alterations, but only 10.8% of them received the recommended treatment. TMB-H was mainly observed in cervical/endometrial cancer, suggesting its potential as a therapeutic biomarker of immune checkpoint inhibitors.

Intra-Patient Genomic Variations of Human Papillomavirus Type 31 in Cervical Cancer and Precancer

Summary: HPV31 variants are predominantly of lineage B in Japanese women with cervical lesions. Deletions in the viral genome occur in the E1 to L1 genes, but retain the E6/E7 genes. High-frequency nonsynonymous nucleotide variations are found in the E1, E2, and E7 genes in cervical cancer samples, indicating positive selection during cervical cancer/precancer development.
