Summary: This study aims to describe the natural history of hepatocellular adenomas (HCAs) and identify predictive factors for HCA evolution after discontinuation of estrogen-based contraception. The results show that weight variation is strongly associated with the radiological evolution of HCAs, and an estrogen exposure score can predict the regression of HCAs.

Summary: In hepatocellular carcinoma treatment, heterogeneity still exists within the Barcelona clinic liver cancer subcategories. A simple model was developed to better discriminate and predict prognosis after resection. This model consistently outperformed the existing BCLC model in terms of discrimination power and prognostication.

Summary: This study examined the prognostic role of tumor burden score (TBS) relative to albumin-bilirubin (ALBI) grade among patients undergoing curative-intent resection of intrahepatic cholangiocarcinoma (ICC). The results showed that decreased tumor burden was associated with better long-term outcomes. Patients with high TBS and high ALBI grade had worse overall survival and higher risk of early recurrence and death.

Summary: This study investigated the roles of CA19-9 and TBS in intrahepatic cholangiocarcinoma, and found that both CA19-9 and TBS were associated with overall survival and recurrence. The combined use of CA19-9 and TBS, evaluated by the CTC grading system, can predict prognosis in these patients.

It found that tumor size, CA19-9 levels, nodal status, margin width, and age had an impact on overall survival. Different margin widths based on patient and disease characteristics may optimize ICC long-term survival.

Management of splanchnic vein thrombosis

Summary: The article discusses splanchnic vein thrombosis, including Budd-Chiari syndrome and portal vein thrombosis. These rare diseases can cause portal hypertension and its complications. Risk factors for thrombosis in these patients include myeloproliferative neoplasms, and a thorough work-up is needed. Long-term anticoagulation is typically recommended. The presence and nature of underlying liver disease affect the management of portal vein thrombosis, with indications for anticoagulation in cirrhosis patients growing. Collaborative studies have provided new insights into the management of these diseases.

Summary: Our understanding of the coagulopathy of liver disease has significantly evolved over the past 25 years. Despite the common impression of hemorrhagic tendency, this form of coagulopathy is actually hypercoagulability. The use of INR as a measure in this situation is not meaningful and may worsen pressure-driven bleeding.

Summary: This study conducted cluster analysis for ICC subtypes to explore their association with long-term outcomes and recurrence patterns. Three distinct ICC subtypes were identified, with significant differences in overall survival and recurrence-free survival. Despite a more favorable disease profile, the inflammatory ICC subtype had a higher incidence of recurrence.
