Converting MMSE to MoCA and MoCA 5-minute protocol in an educationally heterogeneous sample with stroke or transient ischemic attack

BackgroundThe Montreal Cognitive Assessment (MoCA) is psychometrically superior over the Mini-mental State Examination (MMSE) for cognitive screening in stroke or transient ischemic attack (TIA). It is free for clinical and research use. The objective of this study is to convert scores from the MMSE to MoCA and MoCA-5-minute protocol (MoCA-5min) and to examine the ability of the converted scores in detecting cognitive impairment after stroke or TIA. MethodsA total of 904 patients were randomly divided into training (n=623) and validation (n=281) samples matched for demography and cognition. MMSE scores were converted to MoCA and MoCA-5min using (1) equipercentile method with log-linear smoothing and (2) Poisson regression adjusting for age and education. Receiver operating characteristics curve analysis was used to examine the ability of the converted scores in differentiating patients with cognitive impairment. ResultsThe mean education was 5.8 (SD=4.6; ranged 0-20) years. The entire spectrum of MMSE scores was converted to MoCA and MoCA-5min using equipercentile method. Relationship between MMSE and MoCA scores was confounded by age and education, and a conversion equation with adjustment for age and education was derived. In the validation sample, the converted scores differentiated cognitively impaired patients with area under receiver operating characteristics curve 0.826 to 0.859. ConclusionWe provided 2 methods to convert scores from the MMSE to MoCA and MoCA-5min based on a large sample of patients with stroke or TIA having a wide range of education and cognitive levels. The converted scores differentiated patients with cognitive impairment after stroke or TIA with high accuracy.

Collateral Flow in Intracranial Atherosclerotic Disease

Summary: Intracranial atherosclerotic disease (ICAD) is a major cause of ischemic stroke and transient ischemic attack (TIA) worldwide. This article discusses the epidemiology, stroke mechanisms, therapeutics, and prognosis of symptomatic ICAD. It also reviews collateral routes, factors associated with collateral development, and diagnostic imaging modalities in assessing collateral circulation. The associations between collateral circulation and clinical outcomes after reperfusion treatment, as well as the risk of recurrent stroke and functional outcome in ICAD patients on medical treatment, are discussed.

Summary: The COVID-19 pandemic had a significant impact on stroke admissions, IV thrombolysis (IVT), and mechanical thrombectomy volumes, with a decline observed during the first year. However, mechanical thrombectomy volumes remained stable in high-volume centers. These findings suggest a preservation of stroke care for higher severity of disease during the first year of the pandemic.

Summary: This study aimed to evaluate the risks of intracranial hemorrhage and ischemic stroke associated with microbleeds in atrial fibrillation patients. The results showed that the presence of microbleeds increased the relative risk of hemorrhage and stroke. In patients taking a combination of anticoagulant and antiplatelet therapy, the risk of intracranial hemorrhage was higher than that of ischemic stroke.

Summary: Intracranial atherosclerotic disease (ICAD) is a common cause of ischaemic stroke and transient ischaemic attack (TIA) with a high recurrence rate. This review article focuses on cerebral haemodynamics in sICAS and discusses the significance of haemodynamic features in governing the risk of stroke recurrence. The article also covers other clinical implications of these haemodynamic features in sICAS and highlights knowledge gaps and future directions.

Balance Confidence Modulates the Association of Gait Speed With Falls in Older Fallers: A Prospective Cohort Study

Transcriptomic Analysis of Human Skeletal Muscle in Response to Aerobic Exercise and Protein Intake

Summary: This study provides a comprehensive molecular insight into the effects of aerobic exercise, protein intake, and their combination on human skeletal muscle. The results show that aerobic exercise and protein intake impact skeletal muscle through analogous pathways, including aerobic respiration, mitochondrial complexes, extracellular matrix remodeling, metabolic process, and immune/inflammatory responses. Compared to protein intake alone, aerobic exercise combined with protein intake further promotes protein turnover and synthesis, but suppresses skeletal muscle contraction and movement.

Three classes were identified based on caregivers' reports: Overall Frail, Nonambulant, and Robust. Although the prevalence of fulfilling all indicators of successful ageing was low, these findings can help care professionals understand the heterogeneity of partial successful ageing in this vulnerable cohort and provide targeted interventions for healthy longevity.

Summary: This study investigated the impact of EVT technical procedures on outcomes in patients aged 80 years and older, with thromboaspiration technique being associated with better reperfusion outcomes in the elderly population. Further studies are needed to confirm these results and evaluate the safety and efficacy of EVT in older adults.
