Background: The aim of this study was to determine if the achievement of control targets in patients with type 2 diabetes was associated with personal socioeconomic factors and if these associations were sex-dependent. Methods: This cross-sectional, population-based study was conducted in Spain. Glycated haemoglobin (HbA1c) level and other clinical parameters were obtained from electronic primary care records (n = 32,638 cases). Socioeconomic status was determined using education level and yearly income. Among patients, having their HbA1c level checked during the previous year was considered as an indirect measure of the process of care, whereas tobacco use and clinical parameters such as HbA1c, low-density lipoprotein cholesterol (LDL-c) and blood pressure (BP) were considered intermediate control outcomes. General linear mixed effect models were used to assess associations. Results: The achievement of metabolic and cardiovascular control targets in patients with type 2 diabetes was associated with educational level and income, and socioeconomic gradients differed by sex. The probability of having had an HbA1c test performed in the previous year was higher in patients with lower education levels. Patients in the lowest income and education level categories were less likely to have reached the recommended HbA1c level. Males in the lowest education level categories were less likely to be non-smokers or to have achieved the blood pressure targets. In contrast, patients within the low income categories had a higher probability of reaching the recommended LDL-c level. Conclusions: Our results suggest the presence of socioeconomic inequalities in the achievement of cardiovascular and metabolic control that differed in direction and magnitude depending on the measured outcome and sex of the patient. These findings may help health professionals focus on high-risk individuals to decrease health inequalities.

Has Arthroscopic Meniscectomy Use Changed in Response to the Evidence? A Large-database Study From Spain

Summary: This study investigated the use of arthroscopic meniscectomy in Spain from 2003 to 2018 and found an increase in the number of procedures performed during this period, particularly among older patients. The study showed that this increase is unnecessary, as previous research has shown that surgery is not superior to nonoperative treatment.

Summary: This study evaluated the efficacy of treatment for non-suicidal self-injury (NSSI) through a systematic review and meta-analysis. The findings suggest that therapies specifically aimed at reducing NSSI are effective in reducing both NSSI and depressive symptoms, while increasing global functioning.

Redo DAIR: The Game Is Seldom Worth the Candle

Summary: Debridement, antibiotics and implant retention (DAIR) is commonly used for acute prosthetic joint infections, but its outcomes can be inconsistent. Performing a second DAIR after a failed first debridement is controversial, as it has uncertain outcomes and unfavorable prognosis if failure occurs. This study analyzed 84 cases of acute prosthetic joint infection treated with DAIR, and found that the success rate of a second DAIR after failure was significantly lower than the first procedure. The final outcome of the second failed DAIRs was unfavorable with no eradication of the infection. Therefore, removal of the components should be considered.

Summary: Celiac disease is strongly associated with HLA-DQ, specifically with haplotypes DRB1*03-DQA1*05:01/DQB1*02:01 (DQ2.5), DRB1*07-DQA1*02:01/DQB1*02:02 (DQ2.2), DRB1*11-DQA1*05:05/DQB1*03:01 (DQ7.5), and DRB1*04-DQA1*03:01/DQB1*03:02 (DQ8). The distribution of these risk haplotypes in patients with celiac disease varies in different geographical areas.

Summary: This article studies the quality of life (QOL) of Spanish postmenopausal early-stage breast cancer patients who have finished endocrine therapy (ET), QOL changes after endocrine therapy cessation, and the differences between two endocrine therapy modalities (tamoxifen or aromatase inhibitor [AI]). More QOL information after endocrine therapy cessation is needed.

Quality of life of early-stage breast-cancer patients diagnosed with COVID-19 in the first three waves of the epidemic treated in the Spanish region of Navarre

Summary: This study aimed to describe the Quality of Life (QOL) of breast-cancer patients diagnosed with COVID-19, analyze its evolution, compare the QOL of these patients according to the COVID-19 wave in which they were diagnosed, and examine the clinical and demographic determinants of QOL. The results of the study showed that patients with breast cancer and COVID-19 adapted well to the illness, and the differences between wave-based groups may be due to fewer COVID restrictions, more positive COVID information, and more vaccinated patients in the second and third waves.

Summary: This study aimed to investigate the impact of prehabilitation on the postoperative outcomes of heart transplantation and its cost-effectiveness. The results showed that prehabilitation significantly improved patients' preoperative functional capacity and quality of life, while reducing the rate and severity of postoperative complications, mechanical ventilation time, ICU stay, total hospitalization stay, and the need for transfer to nursing/rehabilitation facilities after discharge. Cost-consequence analysis demonstrated that prehabilitation did not increase the total surgical process costs.

Small area variations in non-affective first-episode psychosis: the role of socioeconomic and environmental factors

Gerardo Gutierrez, Tomas Goicoa, Maria Dolores Ugarte, Lidia Aranguren, Asier Corrales, Gustavo Gil-Berrozpe, Julian Librero, Ana M. Sanchez-Torres, Victor Peralta, Elena Garcia de Jalon, Manuel J. Cuesta

Summary: This study examined the geographical distribution of non-affective first-episode psychosis (NA-FEP) in a northern region of Spain and its association with environmental risk factors such as urbanicity, migration, and deprivation. The results showed that NA-FEP incidence risks were spatially patterned and significantly associated with the migrant population, unemployment, and consumption of anxiolytics and antidepressants. These findings suggest the importance of targeting specific geographical areas for preventive interventions addressing modifiable environmental risk factors for NA-FEP.

Initial opioid prescription characteristics and risk of opioid misuse, poisoning and dependence: retrospective cohort study

Anibal Garcia-Sempere, Isabel Hurtado, Celia Robles, Fran Llopis-Cardona, Francisco Sanchez-Saez, Clara Rodriguez-Bernal, Salvador Peiro-Moreno, Gabriel Sanfelix-Gimeno

Summary: This cohort study conducted in Valencia, Spain, aimed to identify individual and initial prescription-related factors associated with an increased risk of opioid-related misuse, poisoning, and dependence (MPD) in patients with non-cancer pain. The study found that the use of ultrafast opioids, short-acting opioids, and long-acting opioids as initial prescriptions was associated with a higher risk of MPD. Additionally, male sex, younger age, lack of economic resources, and registered alcohol misuse were individual factors associated with an increased risk of MPD.

Summary: The study found that the 2018 risk minimisation measures had limited impact on the utilization patterns of oral retinoids among females of childbearing age in four European countries. Pregnancy still occurred during retinoid use, and pregnant women were still prescribed with these drugs. Regulators, policymakers, prescribers, and researchers need to reconsider implementation strategies to prevent any temporary association between pregnancy and retinoid use.

Seroprevalence of antibodies against SARS-CoV-2 in the Valencian Community (Spain) as of October 2022

Anibal Garcia-Sempere, Estela Gimenez, Hermelinda Vanaclocha, Ramon Limon, Salvador Peiro, David Navarro

Summary: A serosurvey conducted in the Valencian Community, Spain, in October 2022, when BA.5 was the predominant variant, revealed that the seroprevalence of anti-nucleocapsid antibodies (indicative of past infection) was 71.0%, and the seroprevalence of total receptor binding domain antibodies (indicative of past infection or vaccination) was 98.4%. The study also found that 66.7% of the population had hybrid immunity, with lower rates in the elderly population. These findings are important for public health strategies, and a second vaccination booster is recommended for the elderly population.
