Summary: Research on isolated human islets can be reliably translated to the in vivo situation, but concordance may fade out under extreme conditions. Perplexing differences exist between insulin secretion in subjects with Type 2 diabetes and their islets studied in vitro, especially concerning kinetics. Future projects should ensure experimental conditions resemble physiological conditions to maintain the function of normal and diabetic islets.

Summary: Brain glucose hypometabolism and neuroinflammation are early pathogenic manifestations in neurological disorders. Neuroinflammation may also disrupt leptin signaling, an adipokine that centrally regulates appetite and energy balance by acting on the hypothalamus and exerting neuroprotection in the hippocampus. The GK rat model of type 2 diabetes mellitus (T2DM) without obesity effects showed attenuated brain glucose uptake and upregulated leptin receptor, IL-18, and IL-6 gene expression in the hippocampal area.

Summary: The variation in quality between human islet samples is a major problem for research, especially as control material. The assays used to assess the quality of human islets in research are not standardized or comprehensive enough. In this study, using proteomics, we identified the diversity of protein signatures and deviations from normal homeostasis in human-isolated islet samples, with a focus on insulin abundance as a predictor of islet homeostasis and quality.
