Blood metal ion levels are not a useful test for adverse reactions to metal debris: a systematic review and meta-analysis

Objectives Alarm over the reported high failure rates for metal-on-metal (MoM) hip implants as well as their potential for locally aggressive Adverse Reactions to Metal Debris (ARMDs) has prompted government agencies, internationally, to recommend the monitoring of patients with MoM hip implants. Some have advised that a blood ion level > 7 mu g/L indicates potential for ARMDs. We report a systematic review and meta-analysis of the performance of metal ion testing for ARMDs. Methods We searched MEDLINE and EMBASE to identify articles from which it was possible to reconstruct a 2 x 2 table. Two readers independently reviewed all articles and extracted data using explicit criteria. We computed a summary receiver operating curve using a Bayesian random-effects hierarchical model. Results Our literature search returned 575 unique articles; only six met inclusion criteria defined a priori. The discriminative capacity of ion tests was homogeneous across studies but that there was substantial cut-point heterogeneity. Our best estimate of the true area under curve (AUC) for metal ion testing is 0.615, with a 95% credible interval of 0.480 to 0.735, thus we can state that the probability that metal ion testing is actually clinically useful with an AUC >= 0.75 is 1.7%. Conclusion Metal ion levels are not useful as a screening test for identifying high risk patients because ion testing will either lead to a large burden of false positive patients, or otherwise marginally modify the pre-test probability. With the availability of more accurate non-invasive tests, we did not find any evidence for using blood ion levels to diagnose symptomatic patients.

The Psychological Impact of COVID-19 on Arthroplasty Surgeons

Summary: This study investigated the psychological well-being of Canadian arthroplasty surgeons during the peak of the first wave of the COVID-19 pandemic. The results showed that approximately 38% of the surgeons experienced emotional distress and 29% experienced depression. The main factors contributing to psychological morbidity were concerns about loss of income, emotional conflict, and safety worries. However, the majority of surgeons demonstrated insight into the impact of COVID-19 on their emotional health.

Evaluation of Polymerase Chain Reaction in the Identification and Quantification of Clinically Relevant Bacterial Species in Lower Extremity Wound Infections

Double Versus Triple Arthrodesis Fusion Rates: A Systematic Review

Continuous, autonomous subsurface cargo shuttling by nature-inspired meniscus-climbing systems

Summary: Water-walking insects use capillary forces to climb or descend the meniscus between water surface and a solid object by changing body posture, which inspired the development of autonomous aquatic synthetic systems that can shuttle cargo underground between landing and drop-off sites. By controlling the contact angle of a coacervate sac or hydrogel droplet, cyclic cargo shuttling can occur continuously as long as reactants from the surrounding aqueous phase are not depleted, potentially leading to applications in autonomously driven reaction or delivery systems and micro-/milli-robotics.

Accuracy of digital templating in uncemented primary total hip arthroplasty: which factors are associated with accuracy of preoperative planning?

Summary: This study retrospectively reviewed 204 patients (220 hips) who underwent elective total hip arthroplasty (THA) between November 2016 and December 2019. The study analyzed templates from 5 different surgeons and compared the accuracy of preoperative digital templating for different stem designs and surgical approaches. The results showed that the accuracy of femoral component size templating was significantly improved with a single-wedge mid-short stem design. The precision of cup sizing was better in the posterior approach group. Accuracy of templating was found to be dependent on the presence of a calibration marker and the stem design.

Asian American faculty's racialised experiences in Christian higher education

Summary: This study investigated the experiences of Asian American Christian faculty in terms of stereotypes and discrimination, and examined their responses to these experiences. The findings reveal that these faculty members face stereotypes and discrimination at the interpersonal level, as well as the perpetuation of stereotypes and discriminatory practices at the institutional or systemic level.

Relaxation and Aging of Nanosphere Assemblies at a Water-Oil Interface

Summary: The relaxation and aging processes of spherical nanoparticles at a water-oil interface were studied using grazing incidence X-ray photon correlation spectroscopy. It was observed that weak attractions between the nanoparticles resulted in gel-like structures, while the broad size distribution of the nanoparticles inhibited in-plane ordering. Structural rearrangements occurred through intermittent fluctuations rather than continuous cooperative motions.

Suicide Behavior Results From the US Army's Suicide Prevention Leadership Tool Study: The Behavioral Health Readiness and Suicide Risk Reduction Review (R4)
