Osteoarthritis and mortality: A prospective cohort study and systematic review with meta-analysis

Objectives: Osteoarthritis (OA) is a leading cause of disability, but the relationship with premature mortality remains uncertain. We aimed to investigate the relationship between OA and mortality from any cause and from cardiovascular disease (CVD). Methods: Electronic literature databases searches were conducted to identify prospective studies comparing mortality in a sample of people with and without OA. Risk of all-cause and CVD mortality were summarized using adjusted hazard ratios (HRs) for joint specific (hand, hip, and knee) and joint non-specific OA. New data from the Progetto Veneto Anziani (PRO.V.A.) study were, also included. Results: From the PRO.V.A. study (N = 2927), there was no significant increase in mortality risk for participants witfi any joint OA (N = 1858) compared to non-OA (all-cause, HR = 0.95, 95% CI: 0.77-1.15 and CVD, HR = 1.12, 95% CI: 0.82-1.54). On meta-analysis, seven studies (OA = 10,018/non-OA = 18,541), with a median 12-year follow-up, reported no increased risk of any-cause mortality in those with OA (HR = 1.10, 95% CI: 0.97-1.25). After removing data on hand OA, a significant association between OA and mortality was observed (HR = 1.18, 95% CI: 1.08-1.28). There was a significant higher risk of overall mortality for (1) studies conducted in Europe, (2) patients with multi-joint OA; and (3) a radiological diagnosis of OA. OA was associated with significantly higher CVD mortality (HR = 1.21, 95% CI: 1.10-1.34). Conclusions: People with OA are at increased risk of death due to CVD. The relationship with overall mortality is less clear and may be moderated by the presence of hand OA. (C) 2016 Elsevier Inc. All rights reserved.

Efficacy and acceptability of psychosocial interventions in schizophrenia: systematic overview and quality appraisal of the meta-analytic evidence

Summary: Psychosocial interventions play an important role in schizophrenia and early psychosis. However, the literature on this topic is heterogeneous and contradictory. This umbrella review summarizes the findings from multiple meta-analyses comparing the effectiveness of different psychosocial interventions with standard treatment or other active interventions. The results suggest that early intervention services and cognitive behavioral therapy have significant effects on symptom reduction and functional improvement in early psychosis. For schizophrenia patients, cognitive behavioral therapy, psychoeducation, and any family interventions show positive effects on symptom reduction, relapse prevention, and functional improvement.

Association Between Pain and Sarcopenia Among Adults Aged ≥65 Years from Low- and Middle-Income Countries

Summary: This study investigated the association between pain and sarcopenia in older adults from 6 low- and middle-income countries, and identified potential mediators. The results showed that higher levels of pain were associated with an increased risk of sarcopenia. Disability, sedentary behavior, and low mobility were identified as potential mediators.

Summary: This study examined the longitudinal relationship between dynapenic abdominal obesity (DAO) and future fall risk in middle-aged and older individuals from Ireland. The results showed that individuals with DAO at baseline had a higher likelihood of falls occurring during the 2-year follow-up period. Interventions to prevent or reverse DAO may be beneficial for reducing fall risk.

Summary: Engagement in recreational activities has positive impacts on the physical and mental health of individuals with mental health conditions. The study explores the impact of recreational-based volunteering, specifically parkrun, on the health, social, and wellbeing of individuals with mental health conditions. Results show that participating in both running/walking and volunteering at parkrun leads to a stronger sense of community and facilitates meeting new people compared to solely participating in running/walking. These findings highlight the importance of the volunteer aspect in recreational activities for individuals' recovery and well-being.

Sarcopenia is associated with a greater risk of polypharmacy and number of medications: a systematic review and meta-analysis

Summary: Polypharmacy in older adults is associated with negative consequences on muscular function, regardless of medical conditions. This systematic review and meta-analysis found that sarcopenia is significantly associated with a higher prevalence of polypharmacy and increased number of medications. The analysis included 29 studies and showed that individuals with sarcopenia had a higher risk of polypharmacy and used more medications compared to those without sarcopenia. However, variations were observed due to different populations. Future research should investigate the direct contribution of specific medications and the number of medications to the progression of sarcopenia in older adults.

Summary: Pancreatic ductal adenocarcinoma (PDAC) and distal cholangiocarcinoma (dCCA) are aggressive tumors with limited treatment options. They share histomolecular features, making differential diagnosis challenging, but also have significant differences with clinical implications. PDAC and dCCA have different prognosis and oncogenic targets, highlighting the importance of accurate diagnosis and targeted treatments.

All-cause and cause-specific mortality among people with bipolar disorder: a large-scale systematic review and meta-analysis

Summary: The results of this systematic review and meta-analysis show that the risk of premature death in patients with bipolar disorder is not only related to suicide and unnatural causes, but also to physical comorbidities. Therefore, in addition to suicide prevention, promoting physical health and preventing physical conditions in individuals with bipolar disorder may reduce premature mortality risk.

Immunonutrition in Radical Cystectomy: State of the Art and Perspectives

Summary: Preoperative nutritional status is crucial in cancer patients undergoing radical cystectomy, as malnutrition increases the risk of post-surgical complications. Muscle mass loss, a major consequence of cancer-related malnutrition, is associated with readmission, longer hospital stays, and higher mortality rates. Immunonutrition, using specific nutrients to boost the immune system, shows promising evidence in reducing complications after bladder surgery. Further research is needed to determine the optimal approach for improving patient outcomes and establish the effectiveness of perioperative immunonutrition.

Musculoskeletal physiotherapists' discharge practices for people treated with low back pain: A United Kingdom survey

Summary: This study investigated the current practices for discharging people with low back pain (LBP) from physiotherapy services in the UK. Majority of physiotherapists used a shared decision-making and person-physiotherapist goal attainment approach to discharge patients with LBP. Some suggested that using a patient-initiated follow-up or graded discharge approach may be more suitable for patients at higher risk of pain flare or with low self-efficacy.

Summary: This study aimed to investigate whether systematic calibration improves the scoring proficiency of JAMRIS-SIJ and whether contrast-enhancement enhances its performance. The results showed that calibrated readers achieved greater reliability in scoring specific inflammatory and structural lesions. Sensitivity and reliability for scoring inflammatory lesions were higher on fluid-sensitive sequences compared to contrast-enhanced sequences. Therefore, systematic calibration should be implemented before using JAMRIS-SIJ in clinical trials, and it is unlikely that contrast-enhanced MRI will improve the performance of this method.

Cardiovascular safety of zoledronic acid in the treatment of primary osteoporosis: A meta-analysis and systematic review

Summary: This study assessed the cardiovascular safety of zoledronic acid in the treatment of primary osteoporosis. The results showed that in women with primary osteoporosis, zoledronic acid may increase the risk of atrial fibrillation and arrhythmias, but the cardiovascular risk in men with osteoporosis is uncertain.
