EULAR study group on 'MHC-I-opathy': identifying disease-overarching mechanisms across disciplines and borders

The 'MHC-I-opathy' concept describes a group of inflammatory diseases with overlapping clinical manifestations and a strong genetic link to the MHC-I antigen presentation pathway. However, the understanding and treatment of these disorders is limited due to patient heterogeneity and lack of systematic investigation. Therefore, interdisciplinary collaboration is needed to decipher the underlying disease mechanisms.
The 'MHC-I (major histocompatibility complex class I)-opathy' concept describes a family of inflammatory conditions with overlapping clinical manifestations and a strong genetic link to the MHC-I antigen presentation pathway. Classical MHC-I-opathies such as spondyloarthritis, Behcet's disease, psoriasis and birdshot uveitis are widely recognised for their strong association with certain MHC-I alleles and gene variants of the antigen processing aminopeptidases ERAP1 and ERAP2 that implicates altered MHC-I peptide presentation to CD8+T cells in the pathogenesis. Progress in understanding the cause and treatment of these disorders is hampered by patient phenotypic heterogeneity and lack of systematic investigation of the MHC-I pathway.Here, we discuss new insights into the biology of MHC-I-opathies that strongly advocate for disease-overarching and integrated molecular and clinical investigation to decipher underlying disease mechanisms. Because this requires transformative multidisciplinary collaboration, we introduce the EULAR study group on MHC-I-opathies to unite clinical expertise in rheumatology, dermatology and ophthalmology, with fundamental and translational researchers from multiple disciplines such as immunology, genomics and proteomics, alongside patient partners. We prioritise standardisation of disease phenotypes and scientific nomenclature and propose interdisciplinary genetic and translational studies to exploit emerging therapeutic strategies to understand MHC-I-mediated disease mechanisms. These collaborative efforts are required to address outstanding questions in the etiopathogenesis of MHC-I-opathies towards improving patient treatment and prognostication.

Extracellular traps formation following cervical spinal cord injury

Summary: Spinal cord injuries can result in permanent loss of function due to primary and secondary injuries. Extracellular traps, composed of chromatin, granular enzymes, and histones, are known to induce tissue damage and may contribute to secondary injury. This study showed, for the first time, that microglia and neutrophils form extracellular traps in the spinal cord following injury. It is important to determine the mechanisms and kinetics of trap formation to develop interventions for mitigating inflammatory processes and improving functional outcomes after spinal cord injury.

Summary: In this study, hesitancy in patients with autoimmune rheumatic diseases (AIRDs) towards COVID-19 vaccines was investigated. The results showed a decrease in hesitancy, but an increase in concerns over the long-term safety of the vaccine. Patients with idiopathic inflammatory myopathies (IIMs) expressed more skepticism towards vaccine science and had more concerns over long-term safety.

Summary: The altered function of conventional dendritic cells (cDCs) in B27-rats promotes the production of inflammatory CD4+ T cells. Addition of exogenous IL-27 to the cell culture can reduce the production of inflammatory CD4+ T cells and increase the production of regulatory T cells. In vivo treatment with recombinant IL-27 can inhibit the development of spondylarthritis in B27-rats.

Distinct modulation of cellular immunopeptidome by the allosteric regulatory site of ER aminopeptidase 1

Summary: ER Aminopeptidase 1 (ERAP1) is an ER-resident aminopeptidase that plays a role in antigen presentation and may be targeted for cancer immunotherapy. In this study, researchers used an inhibitor to explore the effect of targeting ERAP1 on the immunopeptidome of a human cancer cell line. The results showed significant differences in peptide composition and HLA allele utilization between allosterically inhibited and ERAP1 KO cells, suggesting distinct roles for the allosteric regulatory site of ERAP1 in antigenic peptide selection.

Obesity-Related Knee Osteoarthritis-Current Concepts

Summary: The knee is commonly affected by osteoarthritis, which significantly impacts patients' health and function. Major risk factors include genetics, age, sex, mechanical load, and obesity/metabolic syndrome. Recent studies have revealed the role of obesity and metabolic syndrome not only in increasing mechanical loading but also in causing systemic inflammation, contributing to knee osteoarthritis. The understanding of knee osteoarthritis has evolved to view it as a 'whole joint disease' involving multiple structures. Obesity and metabolic syndrome are associated with increased levels of inflammatory cytokines, adipokines with both protective and destructive effects on cartilage, up-regulation of proteolytic enzymes, and increased levels of free fatty acids and reactive oxygen species. The management of knee osteoarthritis should include optimizing body weight and promoting physical activity. Future therapeutic strategies may consider pharmacological therapy targeting specific molecules involved in obesity-related osteoarthritis, with personalized treatment approaches.

Osteomodulin downregulation is associated with osteoarthritis development

Summary: Abnormal subchondral bone remodeling leading to sclerosis is a main feature of osteoarthritis. Osteomodulin (OMD), a proteoglycan, is associated with the sclerotic phenotype. This study found that OMD binds to RANKL and inhibits osteoclastogenesis, thus regulating bone remodeling and contributing to bone and cartilage homeostasis. Targeting OMD may be a promising personalized approach for osteoarthritis.
