A Simple Emergency Department-Based Score Predicts Complex Hospitalization in Patients with Inflammatory Bowel Disease

Summary: This study identified factors such as tachycardia, hypotension, hypoalbuminemia, Clostridioides difficile infection, and endoscopic colitis as predictors of prolonged hospitalization, inpatient colectomy, or salvage therapy in IBD patients with flare. An emergency department-based score utilizing heart rate and albumin level effectively predicted complex hospitalization, providing prognosis in patient care during uncertain times.

Surveillance for Colorectal Neoplasia in Inflammatory Bowel Disease: When to Stop

Summary: Patients with chronic ulcerative and Crohn's colitis have a higher risk of developing colorectal neoplasia (CRN [dysplasia and cancer]) compared to the general population. Risk factors for CRN include the extent of colitis, cumulative inflammatory burden, family history of colorectal cancer, and primary sclerosing cholangitis. Best practices for preventing CRN involve controlling colonic inflammation, conducting high quality surveillance colonoscopy with or without enhanced imaging techniques, removing visible dysplasia if possible, and considering colectomy for patients with unresectable dysplasia, invisible multifocal low-grade dysplasia, or invisible high-grade dysplasia. The decision to stop dysplasia surveillance should be individualized and based on factors such as age, frailty, comorbid conditions, life expectancy, previous surveillance exam results, and risk factors for CRN.

Summary: A retrospective cohort study compared the safety of TNF-a antagonists and non-TNF biologics in patients with IBD and active or recent cancer. The results showed similar risks of cancer progression and recurrence between the two treatment groups. Therefore, the choice of biologic should be based on IBD disease severity in collaboration with an oncologist.

Histologic Predictors of Clinical Outcomes and Healthcare Utilization in Patients With Ileal Pouch-Anal Anastomosis

Summary: Histologic variables of acute and chronic inflammation were found to be associated with future development of chronic pouchitis, need for biologic or small molecule treatment for chronic pouchitis, and hospitalization in patients with ileal pouch-anal anastomosis. The study suggests that histologic findings may have predictive value for clinical outcomes and the utilization of healthcare resources in these patients. Incorporation of key histologic variables into pouchoscopy pathology reports and closer monitoring of patients with significant histologic findings may be necessary.

Pathogen-Specific Alterations in the Gut Microbiota Predict Outcomes in Flare of Inflammatory Bowel Disease Complicated by Gastrointestinal Infection

Summary: This study found that enteric infection with specific pathogens such as Clostridioides difficile, Escherichia coli subtypes, and norovirus can affect the outcomes of inflammatory bowel disease (IBD). The gut microbiome was associated with the characteristics and outcomes of IBD, and different microbiomes were detected after enteric infections. Clinical variables and 16S rRNA sequencing revealed the impact of the gut microbiome on IBD.

Risk factors for incomplete telehealth appointments among patients with inflammatory bowel disease

Summary: This study retrospectively analyzed telehealth visits at the NYU IBD Center from March 1, 2020, to August 31, 2021. The study found that patients with Crohn's disease, females, and those with less social support were more likely to miss telehealth appointments. Additionally, adults over 80 years old were more likely to miss telehealth appointments compared to those aged 60-70 years. Therefore, telehealth may provide an additional venue for older adults, especially those aged 60-80 years, to complement in-person care.
