Prothrombotic immune thrombocytopenia after COVID-19 vaccination

This study reports 5 cases of prothrombotic immune thrombocytopenia following vaccination with AZD1222. Patients presented with various clinical manifestations, including thrombocytopenia and elevated D-dimer levels. Treatment experience suggests that anticoagulation may be effective, but there is also a risk of unusual and severe thromboembolic events.
We report 5 cases of prothrombotic immune thrombocytopenia after exposure to the ChAdOx1 vaccine (AZD1222, Vaxzevria) against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Patients presented 5 to 11 days after first vaccination. The spectrum of clinical manifestations included cerebral venous sinus thrombosis, splanchnic vein thrombosis, arterial cerebral thromboembolism, and thrombotic microangiopathy. All patients had thrombocytopenia and markedly elevated D-dimer. Autoantibodies against platelet factor 4 (PF4) were detected in all patients, although they had never been exposed to heparin. Immunoglobulin from patient sera bound to healthy donor platelets in an AZD1222-dependent manner, suppressed by heparin. Aggregation of healthy donor platelets by patient sera was demonstrated in the presence of buffer or AZD1222 and was also suppressed by heparin. Anticoagulation alone or in combination with eculizumab or intravenous immunoglobulin (IVIG) resolved the pathology in 3 patients. Two patients had thromboembolic events despite anticoagulation at a time when platelets were increasing after IVIG. In summary, an unexpected autoimmune prothrombotic disorder is described after vaccination with AZD1222. It is characterized by thrombocytopenia and anti-PF4 antibodies binding to platelets in AZD1222-dependent manner. Initial clinical experience suggests a risk of unusual and severe thromboembolic events.

Summary: A growing body of evidence indicates that higher-than-normal mitochondrial DNA content (mtDNAc) is associated with better prognosis in acute promyelocytic leukemia (APL) patients, regardless of tumor burden. This effect is more pronounced in low-risk patients. The multivariate Cox proportional hazard model reveals that high mtDNAc is independently associated with a decreased cumulative incidence of relapse. These findings highlight the potential role of mitochondrial metabolism in APL patients treated with ATRA.

Summary: This study investigates the association between chronic hepatitis C virus (HCV) infection and epigenetic changes and biological age acceleration, as well as the reversibility of these changes after sustained virologic response (SVR). The results show that patients with chronic HCV infection have significant epigenetic age acceleration, which partially reverses after SVR. However, some patients still show incomplete reversal of biological age after treatment.

Summary: Relapse is common in AML patients and has a poor prognosis. Treatment options are limited and understanding the molecular response patterns is challenging. In this study, we analyzed the response patterns of relapsed/refractory AML patients treated with selinexor and identified subclonal patterns. One patient showed a long-term course of remission with selinexor maintenance treatment after relapse following alloHCT. This exploratory study provides insights into the effects of selinexor in AML patients.

Summary: In this registry-based study, the outcomes of allogeneic hematopoietic cell transplantation (allo-HCT) in adult patients with acute lymphoblastic leukemia (ALL) were compared. The study found that the total dose of total body irradiation (TBI) did not influence the survival outcomes for patients, regardless of whether they received a standard 12-Gray or lower 8-Gray dose.

Summary: In this study, the metabolic profile of patients with clinically diagnosed PSP was analyzed using wbMRSI and compared with matched healthy volunteers and PD patients. The results showed that PSP patients had a significant increase in the fractional volume of cerebrospinal fluid and a significant reduction in N-acetyl-aspartate (NAA) in all brain lobes compared to PD and healthy volunteers. This suggests that PSP patients have more neuronal degeneration and cerebral atrophy, and the decrease in NAA in the brain lobes is partially correlated with clinical symptoms.

Adhesion of vessel wall to stentriever during combined technique for mechanical thrombectomy in acute ischemic stroke: A histomorphological study

Summary: This retrospective study aimed to investigate the involvement and role of the stentriever in vessel wall injury by analyzing the composition of adherent tissue to the stentriever during combined aspiration thrombectomy with stentriever. The results showed that parts of the vessel wall were detected in 33% of the samples analyzed, and all specimens revealed fresh thrombus material without signs of organization or atheromatous tissue. No vascular tissue was found in free clots of the aspiration samples.

Pallidal Beta Activity Is Linked to Stimulation-Induced Slowness in Dystonia

Summary: Pallidal deep brain stimulation (DBS) can alleviate symptoms in dystonia patients, but it may also cause movement slowness. This study found that after the cessation of pallidal stimulation, the movement speed of dystonia patients increased over time. The beta oscillations in the pallidum were found to be associated with the variance in movement speed.
