ESTRO ACROP guidelines for external beam radiotherapy of patients with uncomplicated bone metastases

After liver and lungs, bone is the third most common site for cancer metastasis. Majority of bone metastases originate from breast, prostate, lung, kidney, and thyroid cancer. Bone metastases can significantly impact quality of life due to symptoms such as pain and neurologic compromise. Radiotherapy is the standard treatment for symptomatic bone metastases, providing effective pain relief with minimal toxicity.
After liver and lungs, bone is the third most common metastatic site (Nystrom et al., 1977). Almost all malignancies can metastasize to the skeleton but 80% of bone metastases originate from breast, prostate, lung, kidney and thyroid cancer (Mundy, 2002). Introduction of effective systemic treatment in many cancers has prolonged patients' survival, including those with bone metastases. Bone metastases may significantly reduce quality of life due to related symptoms and possible complications, such as pain and neurologic compromise. The most serious complications of bone metastases are skeletal-related events (SRE), defined as pathologic fracture, spinal cord compression, pain, or other symptoms requiring an urgent intervention such as surgery or radiotherapy. In turn, growing access to modern diagnostic tools allows early detection of asymptomatic bone metastases that could be successfully managed with local treatment avoiding development of SRE. The treatment for bone metastases should focus on relieving existing symptoms and preventing new ones. Radiotherapy is the standard of care for patients with symptomatic bone metastases, providing durable pain relief with minimal toxicity and reasonable cost-effectiveness. Historically, the dose was prescribed in one to five fractions and delivered using simple planning techniques. While 3D-conformal radiotherapy is still widely used for treating bone metastases, introduction of highlyconformal radiotherapy techniques such as stereotactic body radiotherapy (SBRT) have opened new therapeutic possibilities that should be considered in selected patients with bone metastases. While the results of interventions show promise, data on their long-term effects and impact on healthcare provider retention are lacking. Further research on prevention and treatment of burnout is necessary.

Recognition of and treatment recommendations for oligometastatic disease in multidisciplinary tumor boards

Summary: This study aimed to analyze the recognition of and treatment recommendations for oligometastatic patients by multidisciplinary tumor boards (MDTs) in order to better understand current treatment patterns. The results showed that more than half of oligometastatic cases were discussed in MDTs, and the majority received a recommendation including the addition of local therapy. This highlights the importance of multidisciplinary decision-making and the need for standardization in MDT discussions.

Summary: Current management of B3 lesions typically involves wide excision, although surveillance may be sufficient for some patients. This study investigates the upgrade risk of B3 lesions diagnosed on core needle biopsy (CNB) or vacuum assisted biopsy (VAB), which is comparable to international literature except for flat epithelial atypia diagnosed on CNB. Based on the observed upgrade rate, a guideline applicable for the Belgian population is proposed.

Patient-reported outcomes versus proxy-reported outcomes in supportive and palliative care: a summary of recent literature

Summary: Proxy-reported outcomes are sometimes used as a substitute for patient-reported outcomes in palliative care due to the difficulty in retrieving patient-reported outcomes. However, recent findings suggest that there are factors influencing the discrepancy between patients' and proxies' reports. Proxy-reported outcomes should be used with caution and considered as a complementary perspective rather than a true substitute for the individual patient's outcome.

Repeat stereotactic body radiotherapy for oligometastatic disease

Summary: This study compared the characteristics and outcomes of patients with oligometastatic disease (OMD) treated with a single course and repeat stereotactic body radiotherapy (SBRT). The results showed that although patients treated with repeat SBRT had shorter progression-free survival (PFS), they had longer overall survival (OS). Therefore, the role of repeat SBRT for OMD patients deserves further investigation.

Efficacy and safety analysis in metastatic cancer patients treated with multiple courses of repeat radiation therapy

Summary: This study analyzed the safety and efficacy profile of radiotherapy in patients treated with multiple repeat courses of irradiation. The results showed that multiple courses of repeat radiotherapy maintain a favorable therapeutic ratio of high response combined with reasonable safety profile. Increasing numbers of radiotherapy courses, type 1 reirradiation, and poor overall health status were associated with worse treatment responses.

Nine years of plan of the day for cervical cancer: Plan library remains effective compared to fully online-adaptive techniques

Summary: This study investigated the impact of season of radiotherapy on the outcomes of head and neck squamous cell cancer patients. The results showed that the season of radiotherapy did not have any significant effect on patient outcomes.

Meta-analysis of 5-day preoperative radiotherapy for soft tissue sarcoma (5D-PREORTS)

Summary: In limb-sparing treatment of soft tissue sarcoma patients, a 5-day course of preoperative radiotherapy results in high local control and favorable R0 margins, with acceptable complication rates, particularly for patients receiving higher biological equivalent doses.
