Comorbidities in rheumatic diseases need special consideration during the COVID-19 pandemic

Comorbidities in rheumatic and musculoskeletal diseases contribute to increased severity and mortality of COVID-19, necessitating the development of a risk score and active management strategies for these comorbidities in RMDs.
Comorbidities in rheumatic and musculoskeletal diseases (RMDs) not only increase morbidity and mortality but also confound disease activity, limit drug usage and increase chances of severe infections or drug-associated adverse effects. Most RMDs lead to accelerated atherosclerosis and variable manifestations of the metabolic syndrome. Literature on COVID-19 in patients with RMDs, and the effects of various comorbidities on COVID-19 was reviewed. The initial data of COVID-19 infections in RMDs have not shown an increased risk for severe disease or the use of different immunosuppression. However, there are some emerging data that patients with RMDs and comorbidities may fare worse. Various meta-analyses have reiterated that pre-existing hypertension, cardiovascular disease, stroke, diabetes, chronic kidney disease, heart failure, lung disease or obesity predispose to increased COVID-19 mortality. All these comorbidities are commonly encountered in the various RMDs. Presence of comorbidities in RMDs pose a greater risk than the RMDs themselves. A risk score based on comorbidities in RMDs should be developed to predict severe COVID-19 and death. Additionally, there should be active management of such comorbidities to mitigate these risks. The pandemic must draw our attention towards, and not away from, comorbidities.

Summary: This study surveyed physicians' knowledge and perceptions of antineutrophil cytoplasmic antibody-associated vasculitides (AAV) through an online questionnaire. The results showed variations in understanding and practices of AAV across different countries, as well as insights into the management of AAV patients with COVID-19. The survey also revealed a lack of consensus on the definitions of AAV and a gap between guidelines and current practice.

Venous Thromboembolism in the Inflammatory Rheumatic Diseases

Summary: Most IRDs have an increased risk of VTE, particularly those with a high-risk triple positive APL profile. The duration of anticoagulation after a VTE event is determined by the nature of the event and the presence of underlying thrombophilic states such as APLs. VTE associated with BD necessitates immunosuppression instead of anticoagulation. Caution should be exercised when using JAKinibs in individuals with a history of VTE events. Ongoing post-marketing surveillance studies of various JAKinibs will provide further clarification on the VTE risk associated with these medications.

Summary: In this study, hesitancy in patients with autoimmune rheumatic diseases (AIRDs) towards COVID-19 vaccines was investigated. The results showed a decrease in hesitancy, but an increase in concerns over the long-term safety of the vaccine. Patients with idiopathic inflammatory myopathies (IIMs) expressed more skepticism towards vaccine science and had more concerns over long-term safety.

New-onset systemic vasculitis following SARS-CoV-2 infection and vaccination: the trigger, phenotype, and outcome

Summary: The COVID-19 pandemic has overwhelmed healthcare systems and led to the emergence of autoimmune phenomena like systemic vasculitis. The SARS-CoV-2 virus and vaccines have been associated with clinical manifestations resembling various types of vasculitis, affecting different vessels. These virus- or vaccine-induced vasculitides have distinct characteristics from de novo vasculitis, as they respond better to steroid therapy and can even resolve spontaneously. However, there have been no confirmed cases of SARS-CoV-2 infection or vaccination triggering certain types of vasculitis. The impact of immunosuppression and steroid therapy duration should be determined individually.

Diabetes mellitus in rheumatic diseases: clinical characteristics and treatment considerations

Summary: New research has shown that comorbidities in rheumatic diseases (RDs) are associated with diabetes mellitus (DM), and factors such as genetics, inflammation, treatment drugs, and disease duration need further exploration. Certain disease-modifying antirheumatic drugs (DMARDs) can improve glycemic control, while glucocorticoids (GCs) can worsen it, particularly in systemic lupus erythematosus (SLE) and rheumatoid arthritis (RA). The combination of anti-TNFalpha agents with hydroxychloroquine (HCQ) has been found to reduce the risk of DM in patients with RDs. Understanding the impact of antirheumatic therapies on patients with DM and RDs can help tailor treatment and reduce mortality risk.
