Purpose of reviewOsteoporosis and osteoarthritis are different diseases, with differences in risk factors, bone mineral density (BMD), BMI, phenotype, morbidity and mortality. We review new data on the role of bone metabolism in osteoporosis and osteoarthritis.Recent findingsThe insights in the common convergent and divergent risk factors between osteoarthritis and osteoporosis have resulted in new findings on the role of BMD, BMI, falls, genetics and epigenetics in the pathophysiology of both diseases and on the increased fracture risk in osteoporosis and osteoarthritis. The relation between BMD, BMI and fracture risk in osteoarthritis is dependent on the stage, definition and location of the osteoarthritis and method of BMD measurement. It has been suggested that osteoarthritis should be further specified in terms of bone involvement.SummaryThese new findings open the way to better understand the bone subtypes of osteoarthritis (osteoporotic, bone forming and erosive) and the common and different ways bone is involved in osteoporosis and osteoarthritis. Much can be expected from further prospective studies, when taking into account the heterogeneous nature of both osteoporosis and osteoarthritis.

Summary: The simultaneous use of oral glucocorticoids (GCs) and proton pump inhibitors (PPIs) increases the risk of osteoporotic fractures in patients with rheumatoid arthritis (RA), whereas the risk is relatively lower when GCs or PPIs are used alone. Among patients using both oral GCs and PPIs, the risk of fractures does not increase with higher PPI daily dose or duration.

Summary: This study demonstrates that CBCT is comparable to HR-pQCT in quantifying bone microstructural and mechanical parameters, with higher accuracy in predicting mechanical parameters of trabecular bone. CBCT shows promising potential as a high-resolution clinical scanning technique due to its fast scanning time and good accuracy.

The use of oral glucocorticoids and the risk of major osteoporotic fracture in patients with myasthenia gravis

Summary: This study found that the use of oral glucocorticoids in patients with myasthenia gravis did not increase the risk of major osteoporotic fractures based on both a case-control study and pooled analysis. Osteoporosis prevention in myasthenia gravis patients according to clinical guidelines is still recommended.

Summary: This study investigated the association between prevalent morphometric vertebral fractures (VFs) and glucose metabolism status in individuals with T2D, prediabetes, and normal glucose metabolism. The results showed that women with T2D had a lower probability of having prevalent VFs compared to women with normal glucose metabolism, while there was no significant association for men with T2D and participants with prediabetes.

How to implement guidelines and models of care

Summary: In subjects older than 50 years, the presence of clinical risk factors (CRFs) or recent fractures is crucial for identifying cases. Further evaluation including bone mineral density measurement, imaging of the spine, fall risk assessment, and laboratory examination is needed for patients who are at high short and long-term fracture risk. Treatment options include anti-resorptive and anabolic drugs, and a lifelong strategy is necessary to determine the appropriate therapy. Efforts are needed to improve case finding, raise public awareness, and establish national policies on reimbursement for assessment and therapy.

Summary: This study evaluated the value of conventional radiographs and clinical reassessment in diagnosing clinically suspected scaphoid fractures. The results showed that both conventional radiographs and clinical reassessment had limited accuracy in diagnosing scaphoid fractures and cannot be considered as directive diagnostic methods.

Summary: Fracture liaison services (FLS) are effective in preventing secondary fractures. This study evaluated whether FLS care is associated with reduced subsequent fracture and mortality risk over a 3-year follow-up period. The results showed that FLS care was associated with a lower mortality risk and lower subsequent major/hip fracture risk in the first year for patients with a major/hip index fracture, but not for patients with a non-major/non-hip fracture.

Summary: This longitudinal study aimed to assess trabecular bone scores (TBS) in institutionalized adults with refractory epilepsy and intellectual disability and study the association between TBS and incident fractures during seven years of follow-up. The results showed a high incidence of fractures and a significant decrease in TBS over the follow-up period, but TBS was not associated with incident fractures.

Summary: In this report, a 34-year-old woman with severe osteoporosis and multiple vertebral fractures after her first pregnancy was treated with teriparatide and zoledronic acid. Significant improvements in bone mineral density, microarchitecture, and strength were observed at the hip, spine, and distal radius, but remained severely impaired compared to healthy controls at 40 months postpartum.
