Summary: Since its initial description, classical Hodgkin lymphoma (cHL) has shown a good prognosis, with the goal of treatment being cure and minimal toxicity. However, older patients (>60 years old) have worse outcomes compared to younger patients, and cure is mostly achievable for those who can tolerate full doses of conventional chemotherapy. This review focuses on the difference in biology between younger and older cHL patients, and explores the impact of frailty and comorbidities on outcomes. The article also discusses the toxicities of conventional chemotherapy in different patient groups and highlights the potential of targeted therapies and checkpoint inhibitors as alternatives for those who cannot tolerate traditional chemotherapy. The treatment of older cHL patients remains an area in hematological research that requires further attention.
