In murine models, the expression of inducible nitric oxide synthase (iNOS) in myocardial infarction (MI) has been reported to be the result of tissue injury and inflammation. In the present study, mRNA expression of iNOS, hypoxia-inducible factor-1 alpha (HIF-1 alpha), and vascular endothelial growth factor (VEGF) was investigated in postmortem human infarction hearts. Since HIF-1 alpha is the inducible subunit of the transcription factor HIF-1, which regulates transcription of iNOS and VEGF, the interrelation between the three genes was observed, to examine the molecular processes during the emergence of MI. iNOS and VEGF mRNAs were found to be significantly upregulated in the affected regions of MI hearts in comparison to healthy controls. Upregulation of HIF-1 alpha was also present but not significant. Correlation analysis of the three genes indicated a stronger and significant correlation between HIF-1 alpha and iNOS mRNAs than between HIF-1 alpha and VEGF. The results of the study revealed differences in the expression patterns of HIF-1 downstream targets. The stronger transcription of iNOS by HIF-1 in the affected regions of MI hearts may represent a pathological process, since no correlation of iNOS and HIF-1 alpha mRNA was found in non-affected areas of MI hearts. Oxidative stress is considered to cause molecular changes in MI, leading to increased iNOS expression. Therefore, it may also represent a forensic marker for detection of early changes in heart tissue.

M. A. Verhoff, M. F. Klinger, M. Kettner, C. G. Birngruber

Summary: Establishing a biological profile is crucial in identifying unknown individuals in advanced stages of decomposition, as seen in a case where a skeletonized corpse was found in a basement. Conflicting age estimation results were resolved through DNA analysis, leading to the identification of a missing woman. The scene, reminiscent of crime movies, highlighted the challenges and importance of osteological examinations in such cases.

Variant interpretation in molecular autopsy: a useful dilemma

Stefanie Scheiper-Welling, Monika Tabunscik, Theresa E. Gross, Tina Jenewein, Britt M. Beckmann, Constanze Niess, Elise Gradhand, Cora Wunder, Peter M. Schneider, Markus A. Rothschild, Marcel A. Verhoff, Silke Kauferstein

Summary: Sudden cardiac death in young individuals may be caused by inherited arrhythmic syndromes. However, the pathogenicity of the majority of genetic variants remains unclear, posing challenges in genetic investigations. Thus, an experienced multidisciplinary team is needed to interpret variant significance, identify high-risk relatives, and develop appropriate forensic guidelines.

Atypical wound trajectory after a tangential pistol shot

Constantin Lux, Mattias Kettner, Jan M. Federspiel, Frank Ramsthaler, Marcel A. Verhoff

Summary: This is a report on the autopsy analysis of a woman who was shot. She sustained three wounds, including a lethal penetrating head wound, a graze wound across the forehead, and a tangential penetrating wound in the thoracoabdominal region. Based on 3D reconstructed CT data, the bullet trajectory would have passed through the abdominothoracic cavities, right lung, and liver. However, the actual trajectory showed a prominent curve without deflection by bone. Possible explanations for this unusual bullet track include the woman's body movement at the time of the bullet impact and the shallow angle of incidence that caused the bullet to deflect intracutaneously. Additionally, the resistance of the soft tissue may have caused the bullet to yaw.
