Association between benzodiazepine outpatient treatment and risk of early seizure recurrence in emergency patients with seizure: A multicenter retrospective study

Summary: Seizures are a common reason for emergency department visits. A study conducted in French emergency departments from January to December 2019 found that starting outpatient BZD treatment in discharged seizure patients did not independently impact the risk of early seizure recurrence within 30 days. Factors independently associated with seizure recurrence included chronic epileptic treatment and having had a focal seizure.

Spitalnik, Papa Alioune Ndour, Olivier Hermine, Eldad A. Hod, Pierre A. Buffet, Pascal Amireault

Summary: This study found that a high proportion of SMEs in RBC concentrates from healthy volunteers correlated with poor transfusion recovery. When perfused through a human spleen, 15% and 61% of long-stored RBCs and SMEs were cleared within 70 minutes, respectively. High initial proportion of SMEs also correlated with high retention of RBCs by perfused human spleen.

Impact of puberty as threshold to differentiate outcome of out-of-hospital cardiac arrest care groups: a nationwide observational study in France

Summary: This study compared the characteristics and outcomes of non-pubescent children, adolescents, and adult patients with out-of-hospital cardiac arrest (OHCA). The study found that puberty as a threshold in international guidelines seems to be relevant, and having a shockable initial rhythm was a key factor for survival in all groups.

Thromboprophylaxis Reduced Venous Thromboembolism in Sickle Cell Patients with Central Venous Access Devices: A Retrospective Cohort Study

Summary: Patients with sickle cell disease who use central venous access devices have an increased risk of venous thromboembolism. This study found that thromboprophylaxis can reduce the risk of venous thromboembolism, and hydroxyurea is associated with a lower incidence. Different types of CVADs are also associated with varying rates of thromboembolism.

Comparison of Patients Infected With Delta Versus Omicron COVID-19 Variants Presenting to Paris Emergency Departments A Retrospective Cohort Study

Summary: This study compares baseline characteristics and in-hospital outcomes of patients with SARS-CoV-2 infection with the Delta variant versus the Omicron variant in the emergency department. The results show that patients infected with the Omicron variant were younger, had a lower rate of obesity, were more vaccinated, had a lower rate of dyspnea, and had a higher rate of discharge home from the ED. Compared with Delta, Omicron infection was independently associated with a lower risk for ICU admission, mechanical ventilation, and in-hospital mortality.

Erythropoietin downregulates red blood cell clearance, increasing transfusion efficacy in severely anemic recipients

Madeleine Casimir, Martin Colard, Michael Dussiot, Camille Roussel, Anais Martinez, Carole Peyssonnaux, Patrick Mayeux, Samantha Benghiat, Sandra Manceau, Anne Francois, Nathalie Marin, Frederic Pene, Pierre A. Buffet, Olivier Hermine, Pascal Amireault

Summary: Red blood cell transfusion is an effective treatment for anemic patients, and the efficacy of transfusion is influenced by various factors. This study found a correlation between severe pre-transfusion anemia and a greater than expected increase in hemoglobin levels following transfusion. Additionally, the study suggests that EPO signaling plays a role in improving post-transfusion red blood cell recovery.

Misdiagnosis of thrombotic microangiopathy in the emergency department: a multicenter retrospective study

Summary: This study aimed to assess the rate of misdiagnosis in TMA patients who visited the emergency department (ED) and evaluate the factors and outcomes associated with ED misdiagnosis. The findings showed that approximately 40% of patients were misdiagnosed when TMA was not mentioned in the ED. On the other hand, patients whose TMA diagnosis was mentioned in the ED received more appropriate care and were admitted to the ICU more quickly. Significant changes in blood markers were observed in both groups during ED and ICU admission.

Management of a Patient with Sickle Cell Disease in an Emergency Room

C. Kassasseya, S. Bressand, M. Khellaf

Summary: Sickle cell disease, a common genetic disorder, leads to various serious consequences in patients, such as arterial obstruction caused by sickle-shaped red blood cells and bone infarcts. The occurrence of acute thoracic syndrome during vaso-occlusive crises can be life-threatening. Morphine is commonly used to relieve pain, but caution should be exercised to prevent addiction. Incentive spirometry, antibiotic therapy, and transfusion exchange are important preventive and treatment measures.
