Congenital diaphragmatic hernia (CDH) is a neonatal pathology in which intrathoracic herniation of abdominal viscera via diaphragmatic defect results in aberrant pulmonary and cardiovascular development. Despite decades of study and many advances in the diagnosis and treatment of CDH, morbidity and mortality remain high, largely due to pulmonary hypertension (PH), along with pulmonary hypoplasia and cardiac dysfunction. In patients with CDH, hypoplastic pulmonary vasculature and alterations in multiple molecular pathways lead to pathophysiologic pulmonary vasculopathy and, for severe CDH, sustained, elevated pulmonary arterial pressures. This review addresses the multiple anatomic and physiologic changes that underlie CDH-associated PH (CDH-PH), along with the multimodal treatment strategies that exist currently and future therapies currently under investigation. Surgical Repair of Congenital Diaphragmatic Hernia After Extracorporeal Membrane Oxygenation Cannulation Early Repair Improves Survival

Duy T. Dao, Carmen M. Burgos, Matthew T. Harting, Kevin P. Lally, Pamela A. Lally, Hong-An T. Nguyen, Jay M. Wilson, Terry L. Buchmiller

Summary: The study indicates that early repair of CDH after ECMO cannulation is associated with improved survival and lower incidence of non-repair compared to delayed surgical correction.

Summary: The study on CDH + BPS patients found that pulmonary sequestration is associated with larger defect sizes, a greater need for ECLS, and higher mortality rates.

Birth weight predicts patient outcomes in infants who undergo congenital diaphragmatic hernia repair

Summary: In CDH patients, LBW is a risk factor for mortality and pulmonary morbidity. Prolonged oxygen requirement and increased length of stay are important considerations when managing this population.

Summary: The study found that early CDH-PH is associated with increased risk for inpatient mortality or oxygen support at 30 days. Infants with moderate CDH-PH and severe CDH-PH had a higher incidence of the primary outcome, while severe CDH-PH was also associated with a higher likelihood of requiring ECLS support.

Summary: This study examines the risk-adjusted survival rates of CDH patients over the past 25 years in long-term participants of the CDH Study Group. The results show that the risk-adjusted and observed-to-expected mortality rates for CDH have improved over time.

Extracorporeal Life Support for Respiratory Failure in Patients With Electronic Cigarette or Vaping Product Use-Associated Lung Injury

Summary: Electronic cigarette or vaping product use-associated lung injury can lead to respiratory failure and death. The role of extracorporeal life support in its management remains unclear. This study analyzed the clinical characteristics and outcomes of patients with electronic cigarette or vaping product use-associated lung injury who received extracorporeal life support. It found that venovenous extracorporeal life support can be an effective treatment option for profound, refractory respiratory failure secondary to electronic cigarette or vaping product use-associated lung injury.

Summary: The implementation of a department-wide research curriculum and infrastructure has effectively promoted academic collaboration and productivity among participants at all levels, including trainees and junior investigators. By providing access to resources like data analysis tools, manuscript and grant writing templates, and a variety of lectures, the initiative has successfully reached over 40 UTSW Surgery trainees and staff, serving as a blueprint for other institutions.

Injury Severity, Arrival Physiology, Coagulopathy, and Outcomes Among the Youngest Trauma Patients

Vikas S. Gupta, Ioannis N. Liras, Myron Allukian, Bryan A. Cotton, Charles S. Cox, Matthew T. Harting

Summary: The study investigated the relationship between rTEG values and mortality in trauma patients younger than 5 years old, finding that while rTEG identified coagulopathy, it was not an independent predictor of death. Admission base deficit, arrival temperature, and head injury severity were identified as independent predictors of mortality in these young trauma patients.

Vikas S. Gupta, Cory Wilson, Elizabeth C. Popp, Siqin Zhaorigetu, Scott Collum, Di Jin, Amir M. Khan, Harry Karmouty-Quintana, Kevin P. Lally, Matthew T. Harting

Summary: This study aimed to investigate the association between congenital diaphragmatic hernia (CDH) severity and differences in oxygenation, airway resistance, and pulmonary mechanics. Using a murine model and patient data, it was found that CDH infants had lower SpO2, airway resistance, and tidal volumes compared to control infants.

Temporal Trends in Racial and Ethnic Disparities in Endovascular Therapy in Acute Ischemic Stroke

Summary: According to this study, there are differences in EVT use and outcomes among racial and ethnic groups in patients with acute ischemic stroke. Although the disparities in EVT use between Black and Asian patients and non-Hispanic White patients have narrowed over time, they still exist. However, the study also found that racial and ethnic underrepresented groups have slightly more favorable outcomes in terms of discharge-related results.
