Echocardiographic Assessment of Pulmonary Hypertension in Neonates with Congenital Diaphragmatic Hernia Using Pulmonary Artery Flow Characteristics

The study suggests that PAAT:ET has potential as a prognostic indicator in neonates with congenital diaphragmatic hernia. This parameter can be used for early assessment of ECMO requirement and mortality risk.
Background: Assessment of pulmonary hypertension (PH) is essential in neonates with congenital diaphragmatic hernia (CDH). Echocardiography is widely established to quantify PH severity, but currently used parameters have inherent limitations. The aim of our study was to investigate the prognostic utility of the index of the pulmonary artery acceleration time to the right ventricular ejection time (PAAT:ET) in CDH neonates assessed using echocardiography. Methods: PAAT:ET values were prospectively measured in CDH neonates on admission, on day of life (DOL) 2 and DOL 5-7. Optimal cut-off values to predict mortality and need for ECMO were calculated and PAAT:ET values were compared between non-ECMO survivors, ECMO-survivors, and ECMO-non-survivors. Results: 87 CDH neonates were enrolled and 39 patients required ECMO therapy. At baseline, PAAT:ET values were significantly lower in ECMO patients compared to non-ECMO patients (p < 0.001). ECMO survivors and ECMO non-survivors had similar values at baseline (p = 0.967) and DOL 2 (p = 0.124) but significantly higher values at DOL 5-7 (p = 0.003). Optimal PAAT:ET cut-off for predicting ECMO was 0.290 at baseline and 0.310 for predicting non-survival in patients on ECMO at DOL 5-7. Conclusion: PAAT:ET is a feasible parameter for early risk assessment in CDH neonates.

Summary: The present study aimed to investigate the suitability of NT-proBNP as a biomarker for predicting extubation failure in critically ill neonates with pulmonary hypertension or ventricular dysfunction. The results showed that there were differences in the absolute values of NT-proBNP before and after extubation, and the Zlog transformation of NT-proBNP could better predict the occurrence of extubation failure.

Percutaneous, ultrasound-guided single- and multisite cannulation for veno-venous extracorporeal membrane oxygenation in neonates

Fetal NT-proBNP levels and their course in severe anemia during intrauterine treatment

Summary: The purpose of this study was to investigate the effect of anemia and intrauterine transfusion (IUT) on NT-proBNP concentrations in fetuses with anemia and establish gestational age-dependent reference values of a control group. NT-proBNP levels in non-anemic fetuses are higher than in postnatal life, decreasing with ongoing pregnancy. Anemia severity correlates with circulating NT-proBNP levels. Highest concentrations occur in fetuses with hydrops and with PVB19 infection, respectively. Treatment by IUT leads to a normalization of NT-proBNP concentrations, so the measurement of its levels may be useful in therapy monitoring.

D-Transposition of the great arteries with restrictive foramen ovale in the fetus: the dilemma of predicting the need for postnatal urgent balloon atrial septostomy

Summary: This study retrospectively analyzed prenatal ultrasound and imaging data of fetuses with dextro-transposition of the great arteries (d-TGA) to identify reliable predictive markers for balloon atrial septostomy (BAS) in newborns. They found that a FO diameter < 7 mm, a fixed FO flap, and a hypermobile FO flap were highly significant markers for a restrictive FO. These markers could accurately predict the need for urgent BAS in fetuses with restrictive FO.

Spontaneous breathing in selected neonates with very mild congenital diaphragmatic hernia

Summary: This study explored the feasibility and outcomes of a spontaneous breathing approach (SBA) versus immediate intubation in neonates with prenatally diagnosed very mild congenital diaphragmatic hernia (CDH). The results showed that SBA was successful in most cases, and it was associated with shorter hospital stay, less mechanical ventilation and oxygen supplementation, as well as improved early feeding tolerance compared to standard treatment.
