Summary: In women with PCOS and IR, acupuncture was not more effective than metformin or sham acupuncture in improving insulin sensitivity. However, acupuncture showed fewer gastrointestinal side effects compared to metformin, suggesting it may be a non-pharmacological treatment option with lower risk. Further research is needed to evaluate the combined effects of acupuncture and metformin on insulin sensitivity in these women.

Summary: The aim of this study was to determine if the androgen status modulates the impact of metformin on plasma prolactin levels in women. The results showed that the drug decreased total and monomeric prolactin and increased luteinizing hormone (LH) in control subjects. In women with polycystic ovary syndrome (PCOS), metformin reduced LH, LH/FSH ratio, testosterone, and free androgen index (FAI).

However, it does not have a significant effect on hirsutism score or gonadotropin hormone levels, and is not associated with an elevation in side-effects. When the treatment course is greater than 6 months, combination therapy can effectively reduce FBG and improve insulin resistance.

Summary: The study aimed to observe the effects of MET monotherapy and MET plus LIRA combination therapy on gonadal and metabolic profiles in overweight patients with PCOS. The results showed that both MET monotherapy and MET plus LIRA combination therapy improved glucose metabolism and relieved insulin resistance in overweight patients with PCOS. Furthermore, MET plus LIRA combination therapy was more effective in improving reproductive abnormalities and hyperandrogenemia compared to MET monotherapy.

Summary: This study investigated the expression and DNA methylation levels of eight genes related to endometrial function in the endometrium of PCOS patients and control group. The results showed decreased expression of HOXA10, GAB1, and SLC2A4 genes and increased DNA methylation levels of the HOXA10 promoter in PCOS patients. After metformin and nutritional intervention, improvements were observed in metabolic and clinical variables, accompanied by increased expression of HOXA10, ESR1, GAB1, and SLC2A4 genes and reduced DNA methylation levels of the HOXA10 promoter in PCOS women. These findings suggest that metformin and a carbohydrate-controlled diet can improve endometrial function in PCOS patients by modulating DNA methylation and gene expression.

The primary pathophysiological process in its etiology has not yet been identified, but it is believed to involve insulin metabolism disruption and hyperandrogenemia. This review focuses on the role of insulin secretion, clearance, and decreased sensitivity in target cells as potential primary insults in PCOS pathogenesis, as well as the molecular mechanism behind IR in PCOS.

Summary: This study found that insulin resistance decreases ovarian response in patients with polycystic ovary syndrome undergoing in vitro fertilization, especially in lean individuals. There is an interactive association between insulin resistance and body mass index. Insulin resistance is associated with an increased risk of early miscarriage, but does not significantly impair pregnancy and live birth rate.

Summary: This study aimed to investigate the metabolic characteristics of overweight women with PCOS and analyze the possible mechanisms. The results showed that overweight PCOS patients were prone to hypertension, diabetes, low gonadal levels, and insulin resistance. Inflammatory cytokines CXCL12/SDF-1, Serpin E1/PAI-1, and MIF were predicted to play a crucial role in inducing insulin resistance in overweight PCOS through various biological functions and signal transductions.

Summary: The study demonstrated that REB has renoprotective effects against MTX-induced nephrotoxicity in rats by enhancing antioxidant balance and inhibiting inflammatory response. Moreover, REB activated NRF-2 protein and upregulated SIRT-1 and FOXO-3 genes, while also inhibiting NF-κB-p65 and TLR-4, and activating the mTOR/PI3K/AKT signaling pathway. It attenuated the kidney damage caused by MTX administration.
