Summary: The sinoatrial node (SAN) is important for maintaining proper cardiac rhythm and contraction, but abnormalities within it can cause sinus node dysfunction (SND), leading to increased risk of cardiac arrhythmias. Current treatment for SND is limited to pacemaker implantation, but emerging evidence on genetic abnormalities associated with SND may lead to the development of more effective therapies. This review article examines the anatomy and pathophysiology of the SAN, the epidemiology of SND, and discusses the most common genetic mutations correlated with SND, as well as future research and therapeutic opportunities in this field.

Summary: The erectile function of men with Fontan circulation was comparable to historical controls, but they had decreased levels of sexual desire and overall satisfaction. There was no correlation between study parameters and erectile dysfunction, and a proportion of the cohort reported prior pregnancies in line with population data.

Summary: Referral indications for advanced heart failure care vary between centers, leading to late consideration for therapy or lack of specialist consultation for Fontan circulation-related complications. Guidelines are needed to direct appropriately timed referral. The ACTION Fontan Committee has developed recommended thresholds for advanced heart failure referral to guide primary cardiologists.

Summary: This study investigates the characteristics and predictors of acute and delayed sinus node dysfunction (SND) after ablation of atrial flutter (AFL). The majority of patients with acute SND still require pacemaker implantation despite initial improvement. Close monitoring of patients for at least one year after AFL ablation is reasonable.

Summary: Adult patients with uncorrected CHD exhibit more heterogeneous conduction and variations in preferential SAN exit site compared to pediatric patients, which may be related to aging-related remodeling.

Summary: Aiming at late failure and mortality in adult Fontan patients, we have developed an accurate scoring system to classify Fontan failure and mortality, which requires prospective validation and potential refinement in larger multi-institutional cohorts for risk prediction.

Summary: In patients undergoing the Fontan operation, the presence of an additional ventricle does not significantly impact survival or the onset of complications in the long term. However, the immediate postoperative course may be negatively influenced by the presence of an additional ventricular chamber.

Summary: This study examines the natural evolution of exercise capacity over a 10-year period in Danish Fontan patients and finds that their exercise capacity remains stable during this time. The study also shows that patients with higher levels of physical activity have higher predicted peak VO2 values in exercise tests. This research provides valuable insights for rehabilitation and exercise programs for Fontan patients.

Summary: The study examined FALD in the young adult population post-Fontan surgery and identified non-invasive markers associated with bridging fibrosis. Bridging fibrosis was observed in 35% of patients within the first decade post-Fontan and was linked to worse survival. Changes in platelet counts, even years before biopsy, and APRI/FIB-4 scores had modest discriminatory power in identifying patients with advanced fibrosis. Steatosis may represent an additional risk factor for disease progression in obese patients. Further prospective studies are needed to develop strategies for screening FALD in adolescents.

Summary: Patients with single ventricle heart disease after Fontan palliation are prone to cardiovascular deterioration. A peak oxygen consumption less than 50% of predicted is an independent predictor of Fontan-related morbidity and mortality. The recovery of oxygen consumption after exercise (VO2RD) has not been extensively studied in youth with Fontan physiology.

Summary: This study investigated the self-reported physical and mental health of Fontan patients. The results showed that adult Fontan patients reported worse physical health but better mental health compared to the general population. These findings have important implications for the care of patients with complex congenital heart disease.

Summary: This study analyzed the long-term outcomes of patients post-Fontan procedure and found an increased all-cause mortality in Hispanic population and a correlation between the incidence of FALD and age. This emphasizes the need for specific liver surveillance strategies in patients post-Fontan.

Summary: The study investigates the reliability of HRV as a standalone feature for driver sleepiness detection in a realistic setting, using data from three real-road driving studies. The results show that in realistic driving conditions, subject-independent sleepiness classification based on HRV is poor, indicating a need for further research to control for other influencing factors.

Summary: The paper focuses on deploying machine learning algorithms on resource-constrained edge devices, proposing a new resource-efficient RVFL network. By using density encoding and hyperdimensional computing operations, the method demonstrates higher accuracy on real-world datasets than traditional methods, and can represent the readout matrix in a small integer range, thus improving computational efficiency.

Summary: The use of ICDs in LQTS patients requires careful consideration of individual risk profiles. Risk scores may not accurately predict the risk of appropriate shocks, and patients with low risk scores still experienced shocks. Long QTc and double mutations were associated with a higher risk of appropriate shocks. Patients also faced the risk of inappropriate shocks and complications.
