Screening models using multiple markers for early detection of late-onset preeclampsia in low-risk pregnancy

Background: Our primary objective was to establish a cutoff value for the soluble fms-like tyrosine kinase 1(sFlt-1)/placental growth factor (PlGF) ratio measured using the Elecsys assay to predict late-onset preeclampsia in low-risk pregnancies. Our secondary objective was to evaluate the ability of combination models using Elecsys data, second trimester uterine artery (UtA) Doppler ultrasonography measurements, and the serum fetoplacental protein levels used for Down's syndrome screening, to predict preeclampsia. Methods: This prospective cohort study included 262 pregnant women with a low risk of preeclampsia. Plasma levels of pregnancy-associated plasma protein-A (PAPP-A) and serum levels of alpha-fetoprotein, unconjugated estriol, human chorionic gonadotropin, and inhibin-A were measured, and sFlt-1/PlGF ratios were calculated. All women underwent UtA Doppler ultrasonography at 20 to 24 weeks of gestation. Results: Eight of the 262 women (3.0%) developed late-onset preeclampsia. Receiver operating characteristic curve analysis showed that the third trimester sFlt-1/PlGF ratio yielded the best detection rate (DR) for preeclampsia at a fixed false-positive rate (FPR) of 10%, followed by the second trimester sFlt-1/PlGF ratio, sFlt-1 level, and PlGF level. Binary logistic regression analysis was used to determine the five best combination models for early detection of late-onset preeclampsia. The combination of the PAPP-A level and the second trimester sFlt-1/PlGF ratio yielded a DR of 87.5% at a fixed FPR of 5%, the combination of second and third trimester sFlt-1/PlGF ratios yielded a DR of 87.5% at a fixed FPR of 10%, the combination of body mass index and the second trimester sFlt-1 level yielded a DR of 87.5% at a fixed FPR of 10%, the combination of the PAPP-A and inhibin-A levels yielded a DR of 50% at a fixed FPR of 10%, and the combination of the PAPP-A level and the third trimester sFlt-1/PlGF ratio yielded a DR of 62.5% at a fixed FPR of 10%. Conclusions: The combination of the PAPP-A level and the second trimester sFlt-1/PlGF ratio, and the combination of the second trimester sFlt-1 level with body mass index, were better predictors of late-onset preeclampsia than any individual marker.

Prediction of neonatal coarctation of the aorta at fetal echocardiography: a scoring system

Summary: In a retrospective cohort study at S. Orsola Hospital in Bologna, Italy, 140 fetuses suspected of CoA were evaluated using fetal echocardiography. Arch hypoplasia showed the highest correlation with postnatal CoA, with affected neonates having lower Z scores of the ascending aorta and aortic isthmus. An odds ratio-based multiparametric model was developed to build a scoring system (AUC 0.89) for better identification of affected fetuses.

Meningioma during pregnancy: what can influence the management? A case series and review of the literature

Summary: The timing of surgery for meningioma diagnosed during pregnancy depends on factors like PR + status and impending symptoms such as visual loss or headache. Timing of neurosurgery does not seem to affect the delivery method. A multidisciplinary approach is beneficial for rapid and appropriate diagnosis, as well as better evaluation of the benefits and risks of surgery for maternal and fetal health management.

Impact of Maternal Age on Singleton Pregnancy Outcomes in Primiparous Women in South Korea

Summary: This study aimed to examine the association between maternal age and pregnancy outcomes in primigravidae women in South Korea. The medical data of singleton primigravidae women who gave birth between 2013 and 2018 were analyzed. The results showed that advanced maternal age was associated with increased rates of high-risk pregnancy complications, such as gestational diabetes, preeclampsia, and cesarean delivery. Maternal age was also positively correlated with perinatal morbidity, including preterm birth and low birth weight. Women with advanced maternal age had higher hospitalization rates and more frequent outpatient visits.

Reference ranges of uterine artery pulsatility index from first to third trimester based on serial Doppler measurements: longitudinal cohort study

Summary: This study provides gestational-age specific reference ranges for mean uterine artery pulsatility index (UtA-PI) based on longitudinal data assessment throughout pregnancy. The results show a progressive non-linear decrease in UtA-PI during pregnancy, and the new reference ranges may have better performance in screening for placenta-associated diseases and evaluating the potential risk for pregnancy-induced hypertension and/or small-for-gestational age.

Ultrasound diagnosis of placental and umbilical cord anomalies in singleton pregnancies resulting from in-vitro fertilization

Summary: In this prospective cohort study, we found that placental and umbilical cord anomalies are more common in IVF singleton pregnancies compared to spontaneous pregnancies. We also identified oocyte donor fertilization as an additional risk factor for the development of these anomalies. Ultrasound examinations play a crucial role in screening for these anomalies.

Changes in Artery Diameters and Fetal Growth in Cases of Isolated Single Umbilical Artery

Summary: In fetuses with isolated single umbilical artery (SUA), the diameter of the remaining umbilical artery is significantly larger than in control fetuses. A diameter of <3.1 mm in the remaining artery is associated with a higher risk of fetal growth restriction (FGR). This study highlights the importance of measuring the diameter of the remaining artery to identify fetuses at risk of FGR.

Summary: Noninvasive prenatal testing (NIPT) can detect rare autosomal trisomies (RATs) other than common aneuploidies. However, conventional karyotyping is insufficient for evaluating diploid fetuses with uniparental disomy (UPD) due to trisomy rescue. This study aims to describe the need for additional prenatal diagnostic testing for confirming UPD in fetuses diagnosed with RATs via NIPT and its clinical implications.

Effects of Using Barbed Suture in Myomectomy on Adhesion Formation and Adverse Pregnancy Outcome

Summary: This study compared the postoperative adhesion and pregnancy outcomes between conventional suture (CS) and barbed suture (BS) after minimally invasive myomectomy. The results showed no significant difference in the presence of postoperative adhesion and pregnancy complications between the two suture types. Therefore, the importance of barbed suture in gynecological surgery can be questioned.

Maternal hemodynamic profile during pregnancy and in the post-partum in hypertensive disorders of pregnancy and fetal growth restriction

Summary: This study compared the hemodynamic profiles and cardiac remodeling of pregnant women with hypertensive disorders and either appropriate for gestational age fetuses or growth-restricted fetuses, pregnant women with normotensive pregnancies complicated by fetal growth restriction, and pregnant women with uncomplicated pregnancies. The results showed that women with hypertensive disorders and fetal growth restriction had lower cardiac output and concentric remodeling of the left ventricle during pregnancy, which returned to normal levels during the postpartum period.

IL-17A and Th17 Cells Contribute to Endometrial Cell Survival by Inhibiting Apoptosis and NK Cell Mediated Cytotoxicity of Endometrial Cells via ERK1/2 Pathway

Summary: In this study, the immune status in endometriosis development was investigated. Increased Th17 cell population and IL-17A level were found in the peritoneal fluid of endometriosis patients. IL-17A was shown to enhance endometrial cell survival, inhibit NK cell mediated cytotoxicity, induce HLA-G expression, and promote migration of endometrial cells. These findings suggest that targeting IL-17A could be a potential strategy for endometriosis treatment.

Recurrence, Reoperation, Pregnancy Rates, and Risk Factors for Recurrence after Ovarian Endometrioma Surgery: Long-Term Follow-Up of 756 Women

Summary: The purpose of this study was to evaluate the rates of recurrence, reoperation, and pregnancy after ovarian endometrioma surgery over a 5-year follow-up period. A total of 756 patients were recruited, and 27.9% experienced recurrent endometrioma, with 8.3% requiring reoperation. The risk factors identified for recurrent endometrioma were age <= 31 years, no subsequent pregnancy, and postoperative hormonal treatment <= 15 months.

The cumulative incidence of neonatal metabolic acidemia after terminal bradycardia in the 2nd stage of labor: a survival-based model

Summary: The aim of this study was to estimate the hazard function (HF) for neonatal metabolic acidemia (MA) during the second stage of labor (2STG) and the cumulative incidence function (CIF) for MA based on the duration of bradycardia. The results showed that the duration of terminal bradycardia in the 2STG is associated with a higher CIF for MA.

Hazard and cumulative incidence of umbilical cord metabolic acidemia at birth in fetuses experiencing the second stage of labor and pathologic intrapartum fetal heart rate requiring expedited delivery

Summary: The aim of this study was to determine the cause-specific hazard and cumulative incidence function for umbilical cord metabolic acidemia at birth in patients experiencing the 2nd stage of labor, stratified for FIGO-2015 pathologic intrapartum cardiotocography requiring expedited delivery and duration of 2nd stage of labor. The results showed that pathologic cardiotocography and duration of 2nd stage of labor were associated with an increased risk of umbilical cord metabolic acidemia. Nulliparity, pathologic cardiotocography, and meconium-stained amniotic fluid were significant predictors of umbilical cord metabolic acidemia.
