A Review of the Role of Electronic Health Record in Genomic Research

Electronic health record (EHR)-driven genomic research is a recent strategy used to answer research questions using EHR data linked to DNA samples. In models using EHR, after the subject's DNA is collected, a linkage between the DNA sample and the EHR data is maintained. This makes the EHR the paramount source of phenotypic information. The National Human Genome Research Institute sponsored Electronic Medical Records and Genomics (eMERGE) network began in five sites in 2007 and was expanded to nine sites in 2012. This network has developed the methods and best practices for utilizing EHR as a tool for genomic research. Therefore, it is vital to understand the configuration of EHR used to capture data in clinical practice and feasibility of integration with clinical genetic test results. We present a detailed review of the role and importance of EHR in the field of genomic research.

Noninvasive Venous Waveform Analysis Correlates With Pulmonary Capillary Wedge Pressure and Predicts 30-Day Admission in Patients With Heart Failure Undergoing Right Heart Catheterization

Summary: Background: Heart failure is a leading cause of hospitalization among the elderly, and it is difficult to predict acute congestion in advance. Noninvasive venous waveform analysis in heart failure (NIVAHF) is a novel technology that can monitor volume status and hemodynamic congestion. This study aimed to assess the correlation between NIVAHF and pulmonary capillary wedge pressure (PCWP), as well as the ability of NIVAHF to predict 30-day hospital admission after right heart catheterization. Methods and Results: The study compared NIVAHF with PCWP in 106 patients undergoing right heart catheterization. The NIVAHF algorithm was developed to estimate PCWP. The study found a positive correlation between NIVAHF score and PCWP, and the NIVAHF score could predict 30-day hospital admission with good accuracy. Conclusions: NIVAHF is a noninvasive monitoring technology that can estimate PCWP and predict the risk of hospital admission within 30 days. This measurement has the potential to guide decongestive therapy and prevent hospital admission in heart failure patients.

Understanding the Pathobiology of Pulmonary Hypertension Due to Left Heart Disease

Summary: The development of pulmonary hypertension in patients with heart failure due to left heart disease is common and has adverse prognostic implications. However, there are currently no known specific treatments for this condition. Diagnostic criteria and clinical classification for pulmonary hypertension due to left heart disease are still evolving, posing challenges for researchers. The pathobiology of this condition is not well understood, but factors such as increased wall stress and endothelial dysfunction are believed to contribute to pulmonary vascular remodeling. Further investigation is needed to better understand the drivers of this condition and develop effective prevention and treatment strategies.
