Summary: This study aimed to compare the safety and overall effect of robotic distal pancreatectomy (RDP) to laparoscopic distal pancreatectomy (LDP), particularly in terms of perioperative outcome and short-term oncological outcome. The results showed that RDP was associated with smaller tumor size, higher spleen preservation rate, and lower rate of conversion to open surgery compared to LDP. No significant differences were found in other outcomes between RDP and LDP.

Summary: This study demonstrated that minimally invasive distal pancreatectomy is associated with a significant reduction in major morbidity compared to open distal pancreatectomy, with a linear decrease in postoperative complications as the rate of minimally invasive surgeries increased. The nationwide risk reduction for composite major morbidity after minimally invasive distal pancreatectomy is estimated to be 11% and is expected to improve further with decreasing conversion rates. These findings support the outcomes of recent LEOPARD RCT.

Summary: The study aimed to investigate the effects of using a new innovative endovascular stapler, AEON (TM), on pancreatic leak rates and other outcome measures. Retrospective analysis of prospectively collected data showed that using AEON (TM) led to significantly reduced postoperative drain lipase level, pancreatic fistula rate, and length of hospital stay compared with other commonly used staplers. The results suggest that AEON (TM) stapler may have advantages in reducing complications and should be further evaluated in larger cohorts.

Summary: The experience of surgeons plays a crucial role in achieving better outcomes after high-risk pancreatoduodenectomy. Increasing career volume and years of practice are associated with a decreased rate of clinically relevant pancreatic fistula. Specific operative approaches, such as pancreatojejunostomy reconstruction and omitting prophylactic octreotide, along with minimizing blood loss, are effective strategies for preventing clinically relevant pancreatic fistula.

Summary: The study evaluates the effectiveness of image-guided percutaneous direct pancreatic duct intervention in managing pancreatic fistula after surgery or pancreatitis, when surgical or endoscopic therapy is initially not an option. The results show that in certain cases, direct percutaneous pancreatic duct interventions can be successful in achieving improvement without major morbidity or mortality.
